Army: Psychiatrists Needed on Warfronts

WASHINGTON — U.S. troops on the battlefield found it harder to get the mental health care they needed last year, when violence rose in Afghanistan and new tactics pushed soldiers in Iraq farther from their operating bases.

A report the Army released Thursday recommends sending civilian psychiatrists to the warfront, supplementing members of the uniformed mental health corps.

Surveying a force strained by its seventh year of war, officials found that more than one in four soldiers on repeat tours of duty screened positive for anxiety, depression and other mental health problems. That was comparable to the previous year.

The report found more troops reported marital problems, an increased suicide rate, higher morale in Iraq, but a greater percentage of depression among soldiers in Afghanistan.

"They do show the effects of a long war," said Col. Elspeth Ritchie, psychiatry consultant to Army Surgeon General Lt. Gen. Eric Schoomaker.

Added Maj. Gen. Gale S. Pollock, a deputy surgeon general: "I think the fact that they are doing as well as they are with the demands they are under speaks to a strength and resiliency of the men and women of America."

The report was drawn from the work of a team of mental health experts who traveled to the wars last fall. The experts surveyed more than 2,200 soldiers in Iraq and nearly 900 in Afghanistan.

In the fifth such effort, the team also gathered information from more than 400 medical professionals, chaplains, psychiatrists, psychologists and other mental health workers deployed there.

The recommendation of civilian mental health professionals for battlefield duty is unusual. But civilian contract employees are doing many other jobs in Iraq, from security to providing food service.

The report also recommended longer home time between deployments, more focused suicide-prevention training and insurance coverage for marital and family counseling.

Among other findings were:

_More than 27 percent of troops on their third or fourth combat tour suffered anxiety, depression, post-combat stress and other problems. That compared with 12 percent among those on their first tour.

_Suicide rates "remained elevated" in both Iraq and Afghanistan. There were four in Afghanistan and 34 confirmed or suspected in Iraq. If all are confirmed, it would be the highest rate since the war began.

_The percentage of soldiers reporting depression in Afghanistan was higher than that in Iraq, and mental health problems in general were higher than they had previously been in Afghanistan. The adjusted rate last year for depression in Afghanistan was 11.4 percent, compared with 7.6 percent in Iraq.

Though U.S. troops suffered their highest level of casualties in both campaigns last year, that came as violence was decreasing in the five-year-old Iraq conflict and increasing in Afghanistan, now in its seventh year.

_As fighting against Taliban and al-Qaida fighters in Afghanistan worsened, 83 percent of soldiers there reported exposure to traumatic combat events _ a key factor in the risk for mental health among the troops.

_Having troops spread out and more isolated over the rugged terrain in a less developed Afghanistan occasionally made it more difficult for them to get mental health treatment.

_About 29 percent of soldiers in Iraq said it was difficult to get to mental health specialists for help. That was among troops who had moved from bases to combat outposts set up so they could be closer to the Iraq population. The number among troops not at the outposts who had trouble getting help was only 13 percent.

_Soldiers who had special "Battlemind" training reported fewer problems than those who did not. The program teaches troops and families what to expect before soldiers leave for the wars and what common problems to look for when readjusting to home life after deployment.

_Progress was made toward reducing the fear and embarrassment that keeps soldiers from asking for help with mental health problems. In 2007, 29 percent of those surveyed in Iraq said they feared seeking treatment would hurt their careers, down from 34 percent the previous year.

_Eleven percent of those surveyed in Iraq said their unit's morale was high or very high, compared with 7 percent the previous year. Individual morale was reported high or very high among 20.6 percent, compared with 18.3 percent the previous year.

_In Iraq, some 72 percent of soldiers reporting knowing someone seriously injured or killed.

_Soldiers reported an average of 5.6 hours of sleep per day in Iraq _ significantly less than needed to maintain their best performance _ yet officers appeared to underestimate how it could have that effect.

_Nearly one-third of troops in Afghanistan were highly concerned that they were not getting enough sleep and about a quarter reported falling asleep during convoys last year. Sixteen percent reported taking mental health medications and about half of those were sleep medications.

This is no shock to hear. I have one friend in the reserves, this is his 4th tour to Bush's invasion of power and oil. From what I understand from the people who are there, and have been there, almost no one thinks we should be there, and we have only made things worse for these people. They have less now then when Saddam was in power, and it isnt getting any better. Its getting worse day by day.
Then you have the fact that the solders are ordered that they cant say anything negative about the invasion or Bush. If they do, they can be brought up on court martial charges. Thank Rumsfield for that. He instituted that rule about 6 months after the invasion started. Many solders were talking about why we shouldnt have even gone in much less be there then. So Rummy sealed their lips with threats of life in prison, and that order stands to this day. So they have to hold all this in, and add that to the death and destruction they are in every single day and something has to give. I really do feel sorry for all the men and women of the US armed forces who are forced to be fighting , dying and killing everyday in Iraq for corporate profits.

The focus of this article is the mental and physical deterioration of our troops in the field, and the implicit fact that if they were not bound by human resource shortages, they would be sent state side immediately. The symptoms being outlined are the same one's being reported by all of our allies in the same theatre of engagement, and infact, are identical to those of Russian troops decades earlier.

I believe it is essential that we provide our troops with the moral as well as financial support for life long psychological and psychiatric treatment upon their return to America. Furthermore, I believe that we as a nation should immediately, instruct our politicians to introduce legislation that protects the mortgages, jobs, and famillies of National Guardsmen and women from being rendered destitute while they are forced into far lengthier services than they were ever told they would be serving. And, finally, when you see a soldier at a coffee shop or a restaurant, just say hi, you do not know how much it means to a devestated psyche or soul, to know that not all civillian Americans share the ignorant opinions of the few individuals that have posted negative and insulting comments at this site today. You do not have to agree with war, but do not blame the warrriors, blame the politicians who forced us to go, and remember if it were not for the volunteer army, we would have a draft army.

Specifically for "GlennTurner", you sir/madame, are a pronounced liar, I do not believe that you ever served in uniform, you show no honor, regard for others, or decorum, which is common amongst the ranks, and furthermore, your one sentence, beligerent and offensive postings to each and every article on the topic of military service shows not your intellect but rather your ignorance and inherent lack of humanity whether you are an anti-war activist or not, it is common sense, at least to the majority of the world that troops do not form policy, that is the province of government and the military hierarchy. If the number of those who falsely claimed service in Vietnam were actually there, there would have been no need for the draft.

And, finally, as Dr. Warren teaches us the Bible is our path to build bridges with all cultures, religions, and nations, not to convert them, but to structure a world community that can work together to resolve our differences without war, hatred, enmity, and waste. God Bless the US, Allied, and all the soldiers, civillians, NGO's, and humanitarian organizations working for peace in the world.

You were doing pretty good there, good ideas and you were keeping my interest until you hit the personal attack mode, you may want to pray on that.

Seems to me Mr Turner is not much different from most of the men I served with, an irreverant lot and quite frequently all the things you mentioned such as beligerence and an outward display of lack of regard for others. It's not a truth that everyone in a foxhole is a Christian.

Some of the most vocal opponents of military failures are ex-service members, so why not just say hi to Mr. Turner , just because someone doesn't fit into your idea of what a soldier believes in or how he acts doesn't mean they didn't serve.

Not war-- failure. Duh.

When there's a mismanaged disaster like Iraq, THERE'S A GANDHI IN EVERY FOX HOLE.

But during Desert Storm ( a just and "easy" war) majority of the military was ecstatic and liked their hero welcome.

While it's PC to consider opinions of members of military with respect, most of lower military ranks know very little of whats going on. They got their little corner of the war and usually have no idea of whats going on outside of it ( or even inside of it).

During ALL wars, just and unjust, the destiny of the lower ranks is to suffer, kill and die without question.

It's crude, but thats the nature of military machine.

Indeed, one joins the military to learn to kill, obey orders and die if necessary: good times and bad.

Thats the job, some call it duty.

One can be personally opposed to an unjust war, but obey his/her duty and do heroic deeds to protect their buddies.
Some can't stomach it and bail at the earliest opportunity.
Some are also heroes: become conscientious objectors. They go to jail on the strength of their beliefs.

Then there's the type thats is too cowardly to be called a soldier and face defeat as gracefully as victory. Eh, cdnnewsjunky.

Dear "cdnnewsjunky",
Thank you for your comments, and you are absolutely correct I am honored to have anyone in uniform regardless of gender, race, religion, ethnicity, or nationality. However, there is sir/madame, a bond during combat, an unwritten code, that is never broken, perhaps in camp there is the banter and off-color humour that comes with being in constant mortal danger. But, it is out of bounds, for anyone to sit in the comfort of their home in front of a computer to type out such material that is demoralizing not only to those serving but more importantly to their famillies and loved one's at home. And, finally it is only in the last few years that the FBI, the DOJ, and the armed forces have combined to find literally thousands of so called veterans who have claimed service, medals, and honours, simply to bolster, their own egos and careers - from mayors to ordinary citizens have been revealed as frauds. I apologize if I seemed in any way disrespectful of the code of honour, but we lost 3 Allied and US troops today, and I am in no mood for anyone abusing the sacrifice of the heroes on the front lines. Thank you for your input, I will endeavor to implement your suggestions in my future writing. God Bless.

Pres Bush's bumper sticker has always been 'Support the Troops.' Unfortunately, it is incomplete.

Pres Bush should have been saying, 'Support the Troops, Enlist.' The more Troops we have, the more we can use, the greater time they will have to spend at home, and the Troops will not be on their third and fourth rotations.

Is Sen McCain prepared to say, 'Support the Troops, Enlist. You are needed'?

I want to tell you why Cheney said we are fighting in Iraq. At a meeting just after 9/11 when Cheney was exhorting an invasion of Iraq, someone said, "But none of the attackers were Iraqi. Iraq wasn't involved in 9/11. Most of the attackers were Saudi." Cheney replied, "Yes, but Iraq has better targets."

Depression is a term we're all going to be familiar with in the coming months. The American dream has turned into a nightmare and it took only seven years of the inept, incompetent and corrupt administration of Bush/Cheney to put the "kiss of death" on our economy, democracy and prestige in world affairs. It is difficult to gauge the displeasure of the American people with the failures and incompetence of this administration even though 30% support Bush and his policies which probably include this phony war in Iraq. McCain and his supporters are warmongering to continue the fiasco at all costs to life and treasure. If the republicans gain control of the White House with about the same numbers in congress, then this country will join the scrap heap of former empires that brought about their own destruction through never ending war, corruption in government, emphasis on entertaintments, sports and an uncaring attitude and ignorance of the affairs of state.

Of course they're depressed. The Taliban is growing in leaps and bounds. Our troops don't even know who their enemy is, until he shoots them in the face. Did we not get a clue from the 80's? If Russia couldn't beat them (and is their next door neighbor, even) how the hell can we with what few troops we have there? The result is more air ordinance and more civilians killed fueling more to side with the Taliban. On, but we need 150,000 in Iraq, where there were no "terrists" until they followed us there.

The tactics employed by the Soviets were entirely different from those we have been using since Oct. 2001. They sent large armored divisions into the cities and thought that to control the large population centers was to control the country.

The photo of Iraqi children running away from soldiers really brings our success home huh? The scourge is working!

For our courageous soldiers - all the guys and gals over there....you have no reason to be depressed. Be proud. You have done incredibly difficult jobs and we thank you. We will do ours by voting these incompetent criminals out of the WH in November and get our priorities straightened out over here. Hold fast and know we honor you.
