CHICAGO — At least one in four teenage American girls has a sexually transmitted disease, suggests a first-of-its-kind federal study that startled some adolescent-health experts.

Some doctors said the numbers might be a reflection of both abstinence-only sex education and teens' own sense of invulnerabilty. Because some sexually transmitted infections can cause infertility and cancer, U.S. health officials called for better screening, vaccination and prevention.

Only about half of the girls in the study acknowledged having sex. Some teens define sex as only intercourse, yet other types of intimate behavior including oral sex can spread some diseases.

Among those who admitted having sex, the rate was even more disturbing _ 40 percent had an STD.

"This is pretty shocking," said Dr. Elizabeth Alderman, an adolescent medicine specialist at Montefiore Medical Center's Children's Hospital in New York.

"To talk about abstinence is not a bad thing," but teen girls _ and boys too _ need to be informed about how to protect themselves if they do have sex, Alderman said.

The overall STD rate among the 838 girls in the study was 26 percent, which translates to more than 3 million girls nationwide, researchers with the U.S. Centers for Disease Control and Prevention found. They released the results Tuesday at an STD prevention conference in Chicago.

"Those numbers are certainly alarming," said sex education expert Nora Gelperin, who works with a teen-written Web site called sexetc.org. She said they reflect "the sad state of sex education in our country."

"Sexuality is still a very taboo subject in our society," she said. "Teens tell us that they can't make decisions in the dark and that adults aren't properly preparing them to make responsible decisions."

Cecile Richards, president of Planned Parenthood Federation of America, said the study shows that "the national policy of promoting abstinence-only programs is a $1.5 billion failure, and teenage girls are paying the real price."

Similar claims were made last year when the government announced the teen birth rate rose between 2005 and 2006, the first increase in 15 years.

The new study by CDC researcher Dr. Sara Forhan relied on slightly older data. It is an analysis of nationally representative records on girls ages 14 to 19 who participated in a 2003-04 government health survey.

The teens were tested for four infections: human papillomavirus, or HPV, which can cause cervical cancer and affected 18 percent of girls studied; chlamydia, which affected 4 percent; trichomoniasis, 2.5 percent; and genital herpes, 2 percent.

Dr. John Douglas, director of the CDC's division of STD prevention, said the results are the first to examine the combined national prevalence of common sexually transmitted diseases among adolescent girls. He said the data, now a few years old, likely reflect current prevalence rates.

Disease rates were significantly higher among black girls _ nearly half had at least one STD, versus 20 percent among both whites and Mexican-Americans.

HPV, the cancer-causing virus, can also cause genital warts but often has no symptoms. A vaccine targeting several HPV strains recently became available, but Douglas said it probably hasn't yet had much impact on HPV prevalence rates in teen girls.

The CDC recommends the three-dose HPV vaccine for girls ages 11-12 and catch-up shots for ages 13-26.

Chlamydia, which often has no symptoms but can lead to infertility, can be treated with antibiotics. The CDC recommends annual chlamydia screening for all sexually active women under age 25. Trichomoniasis, also treatable with antibiotics, can cause abnormal discharge and painful urination. Genital herpes can cause blisters but often has no symptoms. It's not curable but medicine can help.

The CDC's Dr. Kevin Fenton said given the dangers of some STDs, "screening, vaccination and other prevention strategies for sexually active women are among our highest public health priorities."

Douglas said screening tests are underused in part because many teens don't think they're at risk, but also, some doctors mistakenly think: "Sexually transmitted diseases don't happen to the kinds of patients I see."

Teens need to hear the dual message that STDs can be prevented by abstinence and condoms, said Dr. Ellen Kruger, an obstetrician-gynecologist at Ochsner Medical Center in New Orleans.

"You've got to hammer at them," with appropriate information at each stage of teen development to make sure it sinks in, she said.

She said there are a lot of myths out there, too _ many sexually active teens think the withdrawal method will protect them, or that douching with Coca-Cola will kill STD germs.

Dr. Margaret Blythe, an adolescent medicine specialist at Indiana University School of Medicine, said some doctors hesitate to discuss STDs with teen patients or offer screening because of confidentiality concerns, knowing parents would have to be told of the results.

Blythe, who heads an American Academy of Pediatrics committee on adolescence, noted that the academy supports confidential teen screening.

I am politically active on local public health issues. As a small player, I have absolutely no interest in big pharma or the US National Government. I do my best to disassociate myself from the aforementioned power institutions.

On the local level, I learned that the greatest return on public investment comes through sex education that includes copious birth control and STD information. The great US Fed and righty regilios only fund disseminating information regarding abstinence. As usual, such policy from the earthly powers, only brings more misery to our unfortunate world.

With respect to vaccines, the cervical cancer vaccine (Gardasil) is the first vaccine ever designed to prevent a cancer. The vaccine should be administered at a young age. Unfortunately, the US Feds and righty regilios insist administering the vaccine to a young female will induce that individual to become promiscuous. This means the vaccine is unavailable to many.

The entire situation is a mess beyond comprehension. It seems the Fed and righty regilios are ecstatic with respect the the amount of grief they can convince the masses to accept.

This alarming sounding statistic is a trifle misleading. Even though 3 million young women carry HPV, only about 10,000 a year get cervical cancer and 4,000 die from it. Furthermore, carrying HPV is not a sexually transmitted disease in the sense that most people don't even have symptoms. On top of that, only a few of the many subtypes of the virus cause cancer. It's a problem, but it's not really all that alarming unless you really like being alarmed. All this info is easily googled. Check out the CDC's info sheet on HPV and the Wikipedia entry for cervical cancer.

Could it be that the medical research community really wants to drum up public support for funding, and the press likes really alarming headlines?

Of course it's a big deal,. My remarks were made in the context of this article, meaning only a tiny percentage of those 3 million girls will end up getting cervical cancer. To imply that they have an STD and are in danger of getting cancer is the worst kind of fear-mongering.

Only 4000 cervical deaths?. To this person these people are statiscally insignificant. I have a terminal form of blood cancer and will leave a wife,four sons and daughter, ages 5-14. 4000 deaths don't just affect 4000 human beings.

Condoms are legal and relatively inexpensive .... Abstinence would be a cure....Hey here is a novel idea, a little parental control here... Teen sex should not be the accepted norm.....Watch out for the Obamaciders as this could be construed as racist as the highest prevalence is among blacks with nearly 50 % having at least 1 std.

While it is very important to protect the health of young women against STDs, I am not willing to trust this study, until further research is done. The study involved 838 girls in 2003-04; hardly enough to give one confidence. And let us not forget that there are powerful forces in Big Pharma who have been pushing the new HPV vaccine. Big Pharma cultivates friends at CDC, you can bet.

I am not one of those who is opposed to vaccines as such. I am 62, and I am grateful that my parents rushed me out to get the polio vaccine asap. However, it seems that there is a vaccine for many things that were once regarded as easily survivable childhood diseases. And one wonders if children are not getting too much too soon. So, prove to us please, that there are no hidden agendas behind this survey and its recommendations.

These stories are misleading. The test group included 18 and 19 year olds. Of the 25% who tested positive for STDs, what percentage were 18 and 19 year olds? 18 and 19 year olds are legally adults, not teens. If the sample was only 14-17 years olds, I bet the findings would be more like 1 in 10. Still a problem, but not the same sensational news story.

Last I hear, the ages of eighteen and ninetten were still considered part of the "teen" years, as is the reason for the suffix "teen."

And I suppose when you teach them to be sexually safe, but its ok to do it, they are somehow smart enough to actually do it? By your comment, you indicate only kids taught abstinence have STD's. I would argue a greater percentage of kids taught about safe sex only have STD's.

In other words, they are not smart enough to practice abstinence, but they are smart enough to practice safe sex? Yeah right.

Both are important, and both should be taught, but only one keeps a bun out of the oven.

Um the diseases were there before the 60's....otherwise there would be no diseases at all. Who knows what the prevalence was before that era, because no studies were done. At least we are armed with information now, and we can combat the diseases with sex education about prevention techniques. The only method that actually increases STDs is religious conservative ideology of "don't have sex until you're married" which has *proven* to be not only ineffective, but actually increases STDs and teenage pregnancy. Duh.

This is the consequence of our present repressive regime. When sex is painted as 'wrong' by a society it invariably creates more problems for the population than are solved. Children rebel, it's the nature of the beast. Well educated children who have the resources to protect themselves are the only sound defense for this epidemic. Interesting comment. AS IF righties, like Larry Craig isn't having sex with strangers -- men no less. Ventner, from Louisiana, with prostitutes, ...the list goes on and on. On both sides of the right/left fence -- including, apparently, Spitzer.


And what to the righties typically propose --- "Just Say No!" abstinence programs which have been conclusively shown not to work. We can WISH all we want that they would work, and if wishes were hybrids, we'd all be driving a brand new Prius'.


Instead we need ever increasing amounts of sexual education -- not just that you can get pregnant, and how to prevent that, but gross, yucky slideshows about STD's. Regular testing in high school, even junior high, to slow down what's become an epidemic.


The alternative? Let the problem get worse, at an ever increasing societal cost??


Or we could go with the approach favored by fundamentalists world-wide, stoning and/or lashing of women who have premarital sex.


America has some serious problems -- violence, drugs, STD epidemics -- unfortunately just condemning people doesn't help solve them.
