The battle for health care reform has intensified, with an actual plan beginning to materialize and key interest and political groups gearing up for a once-in-a-generation legislative fight.

Sources who have spoken with key players in the White House and Congress now say, more definitively than in the past, that the time frame for a bill is October. But what members will ultimately produce is far from clear. Part of the delay is owed to a decision among administration officials to let the process originate on Capitol Hill. Much of it, however, is due to divisions within the Democratic Party over just how far a reform effort should go.

No clearer evidence exists than the salvos launched this past week for and against the creation of a public plan for insurance coverage. The newest Democratic Senator, Arlen Specter, and the consummate centrist, Ben Nelson, both pledged to oppose such a measure, projecting the damage it could cause to the private insurance industry. But one prominent progressive told the Huffington Post that reform without a public plan would be tantamount to "a slap in the face."

Added former DNC chair Howard Dean, in an interview with the Huffington Post: "If you don't have a public option it is not worth doing... The idea that insurance companies and Republicans can prevent Americans from having choice is just wrong. And I don't think the American people will put up with it. The vast majority of Americans, including Republicans, believe there ought to be a choice for the American people."

To this point, the Obama White House has been tight-lipped about its preferences. But high-ranking Democrats say that in private discussions, Obama himself has expressed his desire for a public option.

"The president is ahead of some of his own staff members here," said Dean. "I think we are going to have a public health insurance plan because, in the end, it is what the president wants."

One thing that Democratic sources say the White House and Congress are looking closely at is the model Gavin Newsom has constructed in San Francisco, in which a universal program (at a relatively low cost) has been applied through the use of public clinics. Newsom met with administration officials as well as House Speaker Nancy Pelosi this past week and came away convinced that something would have to be done "not in 16 months but in six months." Helping to corral other mayors to support the president's health care priorities, Newsom was appointed chair of the US Conference of Mayors' Task Force on Health Care Reform.

But Newsom expressed concern that the dialogue coming from the Capitol was too nationally focused. "One-sided health solutions don't work," the mayor told the Huffington Post. "People, particularly in diverse communities, have a connection to their communities and to their clinics. Cultural competency needs to be a huge part of this debate and focus, and I don't hear a lot of that up in Washington, D.C."

In testimony on Capitol Hill on Wednesday, Health and Human Services Secretary Kathleen Sebelius touted community health centers as well as various local and state programs that had proven successful in "incentivising" coverage.

Her main detractors came from the Republican side of the aisle, where efforts have increased to oppose a public plan. And while Sebelius remained fairly broad on what a new health care system might look like, her remarks provided an unusually stronger indication of where the administration stands.

"Can you construct an un-level playing field with a public option unfairly competing with private options? You bet," she said. "Is that the intention of the administration or the majority of Congress when they talk about it? I don't think so at all. It can be designed in any number of ways... so if there really is a level playing field that private insurers don't have the advantage of cherry-picking the market and the public plan doesn't have the advantage of undercutting the cost of driving everybody out, it can work very effectively and does work very effectively all across this country."

Look at the poster above.

This is an example that assuring people who already have health care that they get to keep it is a brilliant move. It takes all the steam out of the insurance companys' obvious scare manipulation of public opinion. If the deal is that a citizen will choose annually, private of public, the populace will quickly educate themselves and presto. In a few years everyone will be in.

Oregon for years had a very viable public health care system, the Oregon Health Plan. It still exists, but has been cut to the bone after years of Republican attacks--however, formerly, it worked and it made sure that every child, every mother, every father, every single adult, everybody had access to medical and dental care. There was a monthly co-pay based on a person's calculated ability to pay, some paid nothing, some paid $10, some paid $100--but EVERYONE had coverage.

The Rethugs bled it dry, not the people who used it.

People give Democrats the power, WH, Congress, everything. What kind of POTUS, Congressional leaders are you? There is so little conviction on your own belief; let someone else try if it is too hard.

First they screwed homeowners by not allowing bankruptcy relief and to do the banks' bidding. Now they are going to screw all Americans to do the Insurance companies' bidding. There is a bad pattern here!

Not only do we need to get all Republicans in the Congress, but be careful of the Republican­s-in-Democ­rats clothing some of these hick Senators are proving themselves to be.

Maybe it's time to have a constitutional convention and reform the Senate. Either get rid of it or elect Senators by proportional representa­tion--just like in the House.

Crap! I'm glad communities feel close to their clinics, but it they fashion a public program for low income, it will not get support from the middle class. People want a medicare for all type of plan 100% paid for with federal tax dollars. Then we want government to butt out. We can modify from there if we need to, but it is where we start.

Wish you would go to the PBS website and watch Frontline's special on how the rest of the world handles the crucial health of its citizens. You say you want medicare for all, but want the governmen to "butt out". Hmmmm. We don't have to re-invent the wheel. There are many fine examples of health care around the world. Ours sucks the most in that it costs the most and we get the least.

People WITH health care insurance are dying because of the poor levels of care we are receiving. Shouldn't we be focused on THAT before more people are given insurance, ensuring that more people will die due to blatant corporate and governmental negligence when the drug companies are producing drugs that the FDA is approving that are killing thousands if not millions of us already??

Most of the educated posters here want single-payer health care. Like medicare or medicade. We want to get rid of middle-men and for-profit health care. The health of our citizens is to vital a national treasure to leave it in the marketplace.

The timing of this is very important. We have to get Specter in line and we have to get Franken seated and up to speed. So, October sounds good. But we also have to get a bill out there early and we have to get people comfortable with it. We have to have a differently level of debate on this than on anything else. We have to do a complete committee process but we still have to move it. If we are forced to do reconciliation we will get the bill, but it will lapse in 5 years. That is a bad way to get a good program through. We have to get our people in the room when the bill is written.

Let's say Universal Health Care is implemented, regardless of format, what will people do and say when many of us say "I told you so and tried to warn you" when people finally wake up to the reality of the fact that, because more people are covered, more people or even the same number of people are getting sick and dying because no one thought to say a word beyond a few of us about the fact that actual care has not been improved, and we are still being given the same medicine and treatments that are making us sick or killing us?

I have read most of the healthcare plans out there and far and away the most rational, comprehensive plan is put forth by Dr. Ezekiel Emanuel in his easy-to-read paperback book - Healthcare, Guaranteed.

His Guaranteed Healthcare Access Plan is the best combination of public oversight and public delivery of healthcare. It:

1) insures everyone
2) puts effective cost controls in place
3) pays for itself with dedicated funds
4) takes healthcare off the backs of businesses
5) delivers high-quality, coordinated care
6) has the framework for reasonable resolution of disputes

It's a simple, rational plan that does not put band-aids on the current dysfunctional, broken system. It will take courage for Congress to take a bold step, but this is the time.

Anyone interested in true healthcare reform should read the plan (in paperback form): Healthcare, Guaranteed by Dr. Ezekiel Emanuel. If it makes as much sense to you as it does to me, write your representatives in Congress about it and demand real reform.

So it won't cost me a cent? And that is an estimate by the government that has $2,000 hammers, 300% cost overrun on the big dig, Medicare that was to cost $50 Billion and really costs $500 billion. A government that told us 5 years ago we needed $70 billion to finish the Iraq war and we are still there. The government that can not afford to take care of our veterans and is looking to outsource their care.

For too long the argument against universal health care has been that "you don't want the government deciding what kind of treatment you need." Why then do we want insurance companies, whose goal is profit for the stockholders - not human health, to decide what kind of treatment we get?

Yesterday I found out that our insurance carrier, BC/BS, is instituting pre-authorization for certain prescription medication. If your doctor thinks you need a particular medication, and it's on a list, s/he will have to call the insurer before they will cover a prescription. It has nothing to do with human health, it's because even the insurance companies don't want to pay the outrageous cost of prescription medication.

I spoke to a man this morning who is on Medicare. He pays $90 a month for his supplemental. He says he has wonderful coverage--better than he had before he turned 65 and paid $600 a month for. Gets whatever he needs. Goes t any doctor he wants. The government does NOT interfere in his care. That is just another gop l ie, manufactured to prevent middle class and poor people from getting care.

What with the stimulus package, TARP I and TARP II, the 2010 bloated budget, bailing out the auto industry, making mortgage payments for people who bought a bigger house than they could afford and whatever else Obama has promised, I don't think there is any money left.
