As a deadline for introducing a health care reform bill approaches the progressive community, which has been biting its tongue for weeks if not months, is now making calculated moves to force President Obama's hand.

Several times on the campaign trail candidate Obama called for sustained pressure from the left to enable him to make more ambitious policy decisions in the health care arena. Former DNC Chair Howard Dean echoed these sentiments at a recent panel on the future of health care reform and urged the American people to "hold their [Democratic representatives'] feet to the fire to make sure they behave like Democrats."

In anticipation of a heated ideological debate, the White House is now shifting the parameters of the discussion -- from the philosophical to pragmatic. A report released by the Council of Economic Advisers (CEA) on June 2, stressed the economic necessity of passing healthcare reform as a means of lowering the deficit while increasing gross domestic product.

The study weighed projected costs under the current system against a set of anticipated outcomes if healthcare reform is passed.

One such outcome is the slowing of the annual growth rate of health care costs by 1.5 percent. Accomplishing this, said Christina Romer, chair of the CEA, would generate a 2 percent increase in GDP in 2020 -- compared with a "no-reform baseline" -- and an increase of almost 8 percent by 2030. For the average family of four, these cost reductions would translate to an additional $2,600 in income in 2020 and an extra $10,000 in 2030. It's the equivalent of a "billion dollar bills lying on the sidewalk," said Romer of the existing system.

By framing the debate in terms of economic imperatives the administration seems to be seeking to ward off partisan politics for as long as possible. "You don't see anyone walking out yet,' said David Cutler, Professor of Applied Economics at Harvard University and veteran of the Clinton era healthcare battle.

Maybe so. But progressives are drawing increasingly bold lines in the sand:

"We have polled [Congressional Progressive Caucus] members very carefully in recent weeks and a strong majority will only support comprehensive healthcare reform legislation that includes a public plan option on a level playing field with private health insurance plans," CPC co-chairpersons Lynn Woolsey (D-CA) and Raul Grijalva (D-AZ) wrote in a letter to the Speaker of the House and Senate Majority Leaders in early April.

Speaking to the America's Future NOW! conference, Dean said, "We cannot spend a trillion and half dollars of the taxpayer's money by rearranging the chairs on the deck of the Titanic!" If there's no public option, do nothing! Don't waste our money."

Simply including the words "public option" may not be enough. One alleged compromise put forth by Republicans and some centrist Democrats to scuttle a public plan option, is the notion of a trigger that would mandate its implementation only if the private market fails to meet a certain set of criteria. This model would resemble the Medicare Part D option, which conservatives tout as evidence that there is no need for a public option because the trigger for Medicare Part D has yet to be pulled. However, upon closer inspection, the patients themselves have no control over the criteria that would lead to pulling the trigger if their needs are not met, ensuring that the option of a public plan was never really a viable one, say some health care experts.

"The trigger [under Medicare Part D] would come into play in any county where there weren't at least two private drug plans, but that has never come close to happening. It has nothing to do with containing costs or guaranteeing quality or access or anything that works for patients and consumers," said Jacki Schechner of Healthcare for America NOW.

But even if progressives hold the line and insist on a public option worthy of its name, the question remains, what would such a plan actually look like? Romer, in her final words to a room full of reporters and members of the medical community called on health care practitioners and physicians themselves to pitch in with ideas. "The people who actually know what the reforms [should be] need to stand up," she said.

She might just have an answer. One of those physicians, Dr. Salomeh Keyhani of Mount Sinai hospital in New York, who was speaking on the same panel as Dean, said that the "silent majority" of physicians who support a public plan are poised to do just that.

I can't decide if this is hilarious or pathetic. And I'm a liberal Democrat.

All President Obama has talked about -- aside from the economy -- since he's been elected is HEALTH CARE. He says nearly every week that it is the most important issue for him.

But hey progressives, if you want to spend your time trying to "make the President" do something he's already doing, knock yourselves out. Seriously, knock yourselves out -- preferrably with something that won't cause permanent brain damage. Fools.

It is incumbent upon us to press Congress to make sure they pass reform that we want. Obama can only advise Congress of what he would want, but it is ultimately up to Congress to get it through.

President Obama has stated he is not for the single payer health insurance option except possibly with triggers. Basically, that would take this option off the table. So, I think it is disingenuous to call people fools who are trying to find an option which is best for all of our citizens and not a watered down "health reform". I wish he WOULD get behind the people for a change.

Collectively, the CEOs of the major health insurers rake in around a half-billion dollars per year in personal compensation. As long as private insurers remain in the picture, people covered by insurance have to subsidize the CEOs' compensation, at a few million to over twenty million dollars a pop, per year. Taking these greedy clowns out of the picture could save a half-billion dollars (or more) EVERY YEAR.

So why isn't single payer the ONLY option being considered?

If you completely try to remove the private health insurance companies, they would push back so hard that we wouldn't get any public health insurance option even on the table. Look at what is happening right now, lobbyists are fighting with everything they have. Until we the people either vote out any congressional representative who doesn't represent our interests repeatedly until they get the message OR there is significant campaign finance reform, we have to realize this. Personally, I think there will be a big turnover in candidates for both political parties if they fight the people on this issue. For those republicans who think this would be their great chance to change the numbers dynamics, no. We will just keep trying to get a different democrat to replace the current representative during the primaries.

Having said that, all americans should fight for a single payer health insurance option with no triggers regardless of party. Allow private health insurance as an option for those "who like" their current plan or for supplemental policies.

That is exactly why we push for single payer, but "compromise" on the public option. Once the people see that the government isn't going to eff it up, more will come on board and within a short time it will become a de-facto single payer system.

We need to talk to friends, family and co-workers across the country regardless of political affiliation as to why public single payer health insurance is so important. Let me give an example of why it effects everyone. Let's say you lose your job and then due to all the stress you end up having a heart attack or getting in a car accident and end up in the hospital for weeks. Now you are out of a job, stressed, health compromised and potentially causing bankruptcy for your family due to medical costs. Even if you still had a private health insurance plan, bankruptcy due to medical costs could result. It doesn't matter what party you belong to.

Let's say you are a conservative business that currently participates in health care for your employees. You could remove the continuously rising health care costs and equation from your business model. In fact, since I believe that the private insurance companies should remain in business as an option and to provide various supplemental options, as a business you could use a supplemental insurance policy as incentive to attract the best candidates for a job.

And congress is more likely to pass a bill if Obama pushes for it. If he doesn't want single payer, and he doesn't, they aren't going to do anything to include it. He doesn't even want public health care, he wants unrealistic "triggers" that will never kick in. He gets to look like a hero for supporting public health care, but it'll be a plan that fails miserably.

But you'll be here to defend him then, too.

Unfortunately, we're only talking about health INSURANCE reform. . . once again failing to get to the root of the rapid increase in cost of actual care. Without following the money and looking into why medical supplies, equipment, medicines & actual treatment continue rising in cost far outpacing inflation, why hospital costs continue increasing, and where the hell all the money spent on health care actually GOES, no reform of insurance will be very effective, nor will any savings involved last for long.

GET OVER IT! Health care is not a free-market economy!

There should be no profit motive in health care, since it is not self-regulated by supply and demand! (If it were, drug companies, or any other service, would not be able to increase the price of their drugs without demand decreasing.)

The UK Health care system is not perfect, but NO ONE loses his or her life savings or goes into bankruptcy due to an unexpected illness, and it covers EVERYONE.

Insurance companies and stockholders do not drive the UK system; it's driven by pragmatism: how many people can be helped given the funds available. With insurance companies taking it from the top in the US, it"s no wonder that only 15% of its people have adequate insurance, and it doesn"t cover as well as the UK system.The real problem with getting healthcare reform started is that the people in government (and Obama) do not share the problem. They have full coverage without having to deal with insurance companies. Their perspective is akin to "Let them eat cake."

I totally agree with MDCambridge! He nailed it. There should be NO price tags on human life, especially in a nation which pretends to be compassionate and human. The greedy pharma companies, along with the AMA and the Health Insurance Companies are raking in obscene profits off american misery and should be brought back to earth. I'm counting on Obama!

The insurance companies, big pharma and the health care industry are not the ones who need help. We the American People need help. When is our Congress going to help us, instead of lining their reelection campaign pockets with money. No trigger. No excuses. Single payer or at least a government run competitive health care plan with a minimum of the benefits offered by Medicare.

And while you are at it, make damn sure the entire Congress is moved to the plan immediately.

Please "do that." We are ready to support you and we are the majority. I am on Medicare - this month I turned 65. But the last 10 years of ever increasing premiums just about ruined our savings.

The HealthCare Debate is a test for the Progessive Politicans we have put in office to get the job done.

As Americans i think we need tell theme that this is probably the Biggest Issue that we sent them to

Medicare Part B is a grotesque effort at bankrupting Medicare so that Big Pharma and acolytes can say "see, it doesn't work".

Repeat after me "NO TRIGGERS"!! Eliminate MEDICARE PART B, and restore the government's ability to negotiate for volume discounts with pharmaceutical companies. Any business knows that!

This is one issue I hope the American people will hold Obama's (and all democrats') feet to the fire. No backsliding on health care. Enough with the compromising to assuage conservatives and insurance companies and pharmaceutical companies. Doctors are fed up with the amount of time and effort they have to expend fighting with insurance companies. They want to treat patients. Single payer is the most efficient option. Give its advocates a seat at the table. Add their voices to the mix. Otherwise, you transparently dismiss a very popular and viable option.

Don't underestimate the support the single payer option has. Don't underestimate how fed up most Americans are with wimpy half measures. Deal with this issue head-on or risk getting your asses tossed out of office.
