Speaking at a town hall in Green Bay Wisconsin, President Barack Obama addressed head on what is the hot debate of the day: whether a health reform overhaul should include a public option for health insurance.

"I also strongly believe that one of the options in the exchange should be a public insurance option," Obama declared, in what was one of his most forceful statements of support since the health care debate began. "And the reason is not because we want a government takeover of health care. I've already said, if you've got a private plan that works for you, that's great. But we want some competition. If the private insurance companies have to compete with a public option, it will keep them honest and it will help keep their prices down."

The remarks came just several hours after the American Medical Association said it would oppose a public option for coverage. But in a reflection of just how delicate this debate has become, the 250,000 member physician group largely backtracked from its opposition later in the day.

"Make no mistake: health reform that covers the uninsured is AMA's top priority this year," a clarifying statement from the group read. "Every American deserves affordable, high-quality health care coverage.

"Today's New York Times story creates a false impression about the AMA's position on a public plan option in health care reform legislation. The AMA opposes any public plan that forces physicians to participate, expands the fiscally-challenged Medicare program or pays Medicare rates, but the AMA is willing to consider other variations of the public plan that are currently under discussion in Congress. This includes a federally chartered co-op health plan or a level playing field option for all plans. The AMA is working to achieve meaningful health reform this year and is ready to stand behind legislation that includes coverage options that work for patients and physicians."

While not directly addressing AMA's position on the public plan, the president did offer what seemed to be a subtle dig at the group's seemingly conflicting objectives. AMA's mission has been health care coverage for all, but it has also opposed every major attempt at a systematic overhaul since the FDR administration.

"Doctors didn't get into the medical profession to be bean counters or paper pushers," Obama said in his early remarks, underscoring the bureaucratic problems that exists in the status quo. "They are not interested in spending all their time acting like lawyers or business executives."

Later in the question and answer session, Obama directly addressed the political component of the debate over the public option, gently calling out critiques for misrepresenting the plan's objectives.

"Now, how this debate is evolving in Washington, unfortunately sometimes kinds of falls into the usual politics, right?" the President said. "So, what you've heard is some folks on the other side saying, I'm opposed to a public option because that's going to lead to government running your health care system. Now, I don't know how clearly I can say this, but let me try to repeat it. If you've got health insurance that you're happy with through the private sector, then we're not going to force you to do anything. All we're saying is for the 46 million people who don't have health insurance or for people who have got health insurance like you... let's change some of those incentives so that we get more people getting prevention, more people getting health care to keep them healthy as opposed to just treating them when they get sick. And I think that we can come up with a sensible, commonsense way that's not disruptive, that still has room for insurance companies and the private sector, but that does not put people in the position where they are potentially bankrupt every time they get sick."

The ability of the president to elevate himself above the usual political scrum proved monumentally valuable during the course of the campaign. His appearance at Green Bay on Thursday suggests that the Obama White House thinks it can apply this formula to the health care reform debate. The friendlier turf, they believe, is outside Washington.

Let me be frank-

It was the Americans that rescued my Grandmother in Poland from the Naw-tzeez.
To this day...she stares lovingly at soldiers dressed in uniform.
Her favourite movie...Wh­ite Christmas with Bing Crosby.
She tears up....and whispers..­."the soldiers..­.the soldiers".­..
That is the America she knows.

And that is the America I have fallen in love with.
It's gone way off track a bit.
But America has got to show that same LOVE to itself.
It has neglected its own.

She was driven out of Poland and worked as a cook in Germany.
That is what saved her from the "concentration camps".
She remembers the Americans came on big trucks.
They handed out chocolate and cigarettes.

I am not a doctor, but I have worked in hospitals for 30 years. Here's a radical idea. If doctors and insuance companies believe so strongly in the free market system, then make the actions of hospitals and doctors totally transparent. Make public the infection rate in every hospital and for every physician. Release all malpractice data on doctors and hospitals. Release all state medical disciplinary actions against doctors. Compile data on how successful Hospital A is compared to Hospital B in treating a particular disease in a particular city. Compile data showing how successful Doctor A is compared to Doctor B in treating a particular disease in a particular city. Make public the cost of all medical/su­rgical/lab­/radiologi­cal procdures in for each hosptial in a city. The free market system presupposes informed patients. LET THEM BE TRULY INFORMED. LIFT THE VEIL OFf MEDICINE.

The AMA is right to oppose the Public Option proposal. If the majority of its members support Single Payer, then it should back them. I agree with them that the Public Option will be a poorly funded, low paying sinecure, put out there to make the insurance companies look good and pick up the patients they won't cover. All the huffing and puffing about “Public Option” is designed to make it seem like a victory for “main street” when it is grudgingly included.

Let us hope this is not all hot air. There is strong public desire for an affordable alternative, and if Pres Obama and Congress are using words only intended to fool the voters, that might not be a smart political move. The threshold of patience has been tested with 8 years of Bush and the ongoing WS misbehavior that neither the WH nor Congress appear to be taking seriously.

If that were so, the insurance companies would be jumping on the "public option" bandwagon without any triggers.
This is not the case, they are fighting it tooth and nail, and perhaps grudgingly saying it is OK if there are 'triggers' only.

No, I appreciate your viewpoint.­...don't exactly agree with it. I just find it hard to believe that the Europeans have developed working systems (not perfect) that they would never trade for ours. My Italian relatives enjoy an excellent health care system....­.Italy!!!!­! And all we can do here in the states is worry if some poor fool might cheat the system or that some poor insurance executive might not get a large enough bonus.

I have a unique perspective.
While I attended college, I worked for a decade at the University of Iowa Hospitals & Clinics, a teaching hospital.
Over another decade I have worked for a variety of insurance companies including Physician's Mutual, Mutual of Omaha, Anthem BCBS and United Healthcare.
In college, I watched a stream of Medical Students move through the hospital. When they had their rotations in my department, I got to know--most briefly, others longer--a variety of medical students. They were in it for the money. They thought they were bright and wanted to get rich.
While working for the hospital, I saw that the entire healthcare system was just a big business. The doctors, the hospital administrators, Big Pharma, Baxter medical products--all of them were trying to get rich on the back of the health care system. The hospital tore out old beds and put in new ones and the rooms cost twice as much--$1,0­00.00 a day just for the 3-Hots & a Cot.
Insurance: squeezing as much money out of the premium payers and paying for the littlest amount of stuff.
At one company, all we did was write two versions of the same software: one that helped the insurance company screw the doctors,the other that helped doctors screw the insurance companies.
Every step in the healthcare machine is a transaction, a fee, a little more money squeezed out of the health care industry.
Our ONLY Hope is public funding of healthcare. PERIOD.

I, too, work in the industry briefly and noticed similarities in the story you told.

It's all about the money, and only in the marketing of the product did the welfare of the client/patients were ever mentioned.

I fact, the cost for the most preventable of medical costs were the chiefly responsible for the majority of cost. Poor people in these plans are worth nothing in the real world until they are deathly ill. A young mother on welfare, receiving no prenatal care, with a child born prematurely and in the NIC units all over the nation can reach cost of 200-300k easily.

Can anyone tell me, honestly, that these poorer patients are worth that much money? Society sure doesn't treat them that way.

It's irrelevant. The government already holds a >50% monopoly on all health care spending in this country. IF they could drive down health care costs, they would already be doing so today. Since they can't or won't, there's no reason to believe that they could do so with another 5% market share.

i understand what you are trying to say but there is still a middle man right now and that middle man is the private insurance ...if we eliminate them ...prices will go significantly down ..and When all patients are under ONE system, the payer will have the biggest power..thi­s will create a monopoly on the buying power ...thus forcing cost to remain reasonably under control ...and when you buy into a pool of 300 million people your premiums will be lower ....

so
1-eliminating the extra burden (the private health insurance ) in time of course
2-crating a united front buyer ..this will be like 80% at least to be effective
3-a greater pool means more money and lower premium

The VA already does this, and it delivers health care for much less than Medicare, Medicaid, or private insurers.

The same doctors' group that wants single payer INSURANCE, (while maintaining their for-profit health care system) have said that doing this piecemeal won't work at all. Adding a public "option" won't drive down health care costs whatsoever, so as far as I can tell there are only two outcomes..­.

1) Health care costs continue to skyrocket, and Republicans seize on the failure to dismantle everything Obama did, so we end up back where we started.

2) Health care costs continue to skyrocket, and Obama convinced people that it's because he needs even MORE power, after which we end up with a Medicare-for-all system fundamentally flawed by the same political interests that are bankrupting Medicare today. Due to the Dems' decision to frame the debate around insurance rather than our health care system, the doctors and hospitals are even further entrenched and even less likely to budge.

It also has all patient/tr­eatment/ou­tcomes reporting to once source. What an unparalleled opportunity for a real world analysis of what really works for most people! Personally I see that as the next real opportunity for massive savings AND increased health. A real win win possible only with single payer.
