WASHINGTON — Barack Obama isn't used to hearing boos.

For all the young president's popularity, the response he got Monday from doctors at an American Medical Association meeting was a sign his road is only going to get rockier as he tries to sell his plan to overhaul the nation's health care system.

The boos erupted when Obama told the doctors in Chicago he wouldn't try to help them win their top legislative priority _ limits on jury damages in medical malpractice cases.

But what could they expect? If Obama announced support for malpractice limits, that would set trial lawyers and unions _ major supporters of Democratic candidates _ on the attack. Not to mention consumer groups.

Every other group in the health care debate has a wish list and a top priority. Insurers don't want competition from the government. Employers don't want to be told they have to offer medical coverage to their workers. Hospitals want to stave off Medicare cuts. Drug companies want to charge what the market will bear.

Obama can't give all of them what they want. Instead, he's got to figure what's just enough to keep as many groups as possible on board _ without alienating others. It's a fine line for him _ and sometimes for them.

"It's a coalition issue," said Robert Blendon of the Harvard School of Public Health, an expert on public opinion and the politics of health care. "No major group is able by itself to sink health reform. But if numbers of them come together for different reasons, it could really hurt the direction the president wants to go in."

The doctors were only Obama's first house call. He'll be making his case to the other groups _ and to the nation at large _ in an increasingly energetic campaign to get a bill passed by the end of his first year in office.

AMA insiders shouldn't have been surprised by Obama's upfront refusal to consider malpractice caps.

The group couldn't get that idea passed by a Republican Congress and president a few years ago. Some states have such curbs, but anyone who can count votes knows the chances for national limits are slim to none with Democrats in charge of Congress.

Instead, Obama left the door open to some kind of compromise on malpractice.

The president said he's willing to explore alternatives to taking doctors to court. In the past, he supported special programs in which hospitals and doctors are encouraged to admit mistakes, correct them and offer compensation. Studies have shown the approach can work, because doctors' refusal to acknowledge mistakes is one reason many families file suit.

Doctors have special reasons to be wary of the president's plans to overhaul the health care system.

Not long ago, doctors' decisions were rarely questioned. Now they are being blamed for a big part of the wasteful spending in the nation's $2.5 trillion health care system. Studies have shown that as much as 30 cents of the U.S. health care dollar may be going for tests and procedures that are of little or no value to patients.

The Obama administration has cited such findings as evidence that the system is broken. Since doctors are the ones responsible for ordering tests and procedures, health care costs cannot be brought under control unless they change their decision-making habits.

"Change is scary," said Dartmouth University's Dr. Elliott Fisher, a doctor turned costs researcher. "I think there is a fear of loss of autonomy, that someone is going to tell you what to do." Fisher collaborated on research that showed wild differences in health care spending around the country _ and no signs of better health in the high-cost areas.

But Obama did not blame the doctors. Instead, he tried to woo them, much as he has done with recalcitrant foreign leaders.

"It's the equivalent of international diplomacy. He's got to make them feel like it's possible to have dialogue about what the future looks like," said Blendon. "I think he's starting out with the AMA, but before the summer's over he's going to reach out to a lot of the other groups."

Obama assured the doctors that his plan would provide them with objective information on what treatments work best, with new computerized tools to better manage their patient case loads, and with support for harried solo practitioners to form networks.

He promised that Washington would not dictate clinical decisions. And he asked the doctors to imagine a world in which nearly every patient has insurance coverage and they can devote their full attention to the practice of medicine.

"You did not enter this profession to be bean-counters and paper-pushers," Obama said. "You entered this profession to be healers _ and that's what our health care system should let you be."

That line got him an ovation.

The author fails to mention the many times Pres. Obama was applauded during his speech. I watched as much of the speech as was shown on CNN, and it was the fact that there so much applause that caught my attention. This article only tells of the occasional boos and one ovation, so over half of the audiences responses are left out. This skews the ensuing discussion, regardless of the quality of the comments or the canned responses of others.
Like many others, I'm sure, I miss the work of reporters who actually report, not choose which aspect of an event they will put out to the public.

As a former nursing student who refuses to be an accomplice to hospital greed as well as a legalized drug pusher for BigPharma, it used to be that people only went to the doctor when they were sick. Now you are "sick" until you go to the doctor so that they can make you "well." Drugs are the goldmine and guarded as such.
The healthcare system has people brainwashed into thinking that they are sick. Many sheeple don't brag about being in good health, they brag about how many medications they take and how many doctors they must see.
Something is very wrong with the system. It would have behooved me to be go along and lick my lips at the big money I would soon make instead of seeing the big picture. In my heart of hearts I know that what's going on is wrong, wrong, wrong.

So the doctor`s union (AMA) booed Obama. As Gomer would say: “Surprise, surprise.” To achieve control on runaway increases in health care everyone involved will be required, in current parlance, to “get a haircut.” That will include doctors. Especially those doctors that are not just practicing physicians, but are medical business. Those who have ownership interests in clinics and testing facilities, and thus profit from every test they prescribe, and often over prescribe.

Finally a word about the whole malpractice issue. There is a lot wrong with the way malpractice claims are handled in the courts, this applies to tort litigation in general. The way the system operates currently, when damages are awarded and in what amounts, is far too much of a roll of the dice. And I would be the last to rise to the defense of the major interest group opposing any reform of the system. That is trial lawyers, a group responsible for siphoning so much wealth and productivity out of the American economy. Still doctors seem to expect to be compensated highly, to reflect the highly skilled work they do with the lives and health of patients in the balance. Yet when the screw up they would like the consequences to be that of a $15 per hour dinner cook who burnt your eggs.

Well said. Most people who mention caps cite the woman who was burned by coffee from McDonalds. I put to everyone to take a long hard look at that womans injuries. They were horrific. Should she have put the coffee between her legs? No. And no one was arguing that point. The lady only wanted her medical bills covered. Mickey refused and she took them to court and won big. No caps. There should be consequences if you operate on the wrong side of the brain or removed the wrong leg. I remember getting an MRI done. The technician scanned the wrong leg, then had the nerve to yell at me because I didn't realize she had scanned the wrong leg. He**, I don't work there!

Let's imagine everyone getting a free gold brick from the gubmint too, while we're at it.

I enjoy this vitriol targeted at doctors. I imagine these armchair internet warriors are eerily silent when a doctor (even an AMA one) is about to examine them. You can't force a doctor to study and later practice medicine. What happens when fewer competent resources (doctors) are available to heal a service that has infinite demand (health care)? It's called long queues and disastrous care. Add on illegal immigrants and slothful individuals not wishing to exercise and care for themselves (remember in places like Europe people live healthier lifestyles by dieting and exercise), and you have an absolute disaster.

Yes, and one of the reasons is that the single-payer system PROMOTES a healthy lifestyle by focusing on preventive medicine as a means to cut costs with the added benefit of actually making people healthier. Health care providers try to teach their patients how to live a healthier lifestyle. "Free market" medicine is more concerned with profit, you can't patent and sell a healthy lifestyle but you can patent a sell pills by the dozens. What you don't realize is that we all pay far more for the uninsured to visit the emergency room than it would actually cost to provide preventive care.

Well, I've been in the Dr's office recently and I've felt like booing the doctor myself.

BTW - we have an MD in the family and she was the only one who voted for McCain - she openingly stated that Obama was a secret terrorist. My mild mannered and usually passive sister got her warrior on and railed back with such furor that her son and had to intervene.

Since obstructionistic republicans support the systematic plundering of the middleclass by ruthless corporations like UnitedHealthcare, this is why we need government oversight between the public and greed-driven, parasitic health insurance providers. Profit at the expense of human suffering is a republican engineered abomination. Forget bipartisanship; true non-profit health insurance reform will provide affordable and cost-effective options that include a public offering. Since Georgia is self-insured, state employees have no due process patient protection rights, which means UnitedHealthcare can deny medical care and authorize exorbitant copays and deductibles with no appeal oversight. Real, quality-driven reform will end the monopolistic strangle-hold enjoyed by morally bankrupt health insurance corporations. In Georgia, Cigna and UnitedHealthcare officials colluded with state republicans to eliminate Blue Cross and Blue Shield as a competitor, narrowing the slate of choices to two, with the end result being higher premiums, bigger profit, and reduced benefits. Mr. Obama: The health insurance industry (and the despicable republicans they bankroll) will not retreat quietly from billions in annual profits! I want the same government health insurance option the military and Congress (and their families) enjoy. When compared to the average American, why do politicians live longer? Answer: universal, affordable health insurance.

Please stop smearing the doctor profession. Most of us are in this field because we wanted to help people, and that is what gets us up in the mornings for insanely long often physically strenuous work days. I feel like patients don't understand just the amount of labor that goes into being a responsible knowledgeable doctor...

I know some of you have had bad experiences with the medical profession, but there are bad eggs in literally every profession on earth.

Why do so many doctors support the AMA? One of the biggest impediments to providing healthcare insurance to patients. Their rhetoric does not match their actions. AMA is only concerned with making as much money as they can without regard to patients welfare. You can not have it both ways.

If some of the doctors at the AMA conference we're booing President Obama, that says more about the mentality of those "doctors" than anything else. And it confirms that at least some percentage of that profession is obsessed with greed and personal gain more than the care of their patients.
