WASHINGTON — The drive to remake the nation's health care system suffered yet another setback in Congress on Thursday when a pivotal group of House Democrats demanded changes in legislation the leadership was drafting on a fast track.

The emerging bill "lacks a number of elements essential to preserving what works and fixing what is broken," 40 members of the Blue Dog Coalition of moderate to conservative Democrats wrote party leaders. To win their support, they said, any legislation would need to be much more aggressive in reining in health care costs as well as in addressing a disparity in Medicare payments they said adversely affects rural providers.

A group of the moderates met into early evening with House Speaker Nancy Pelosi and Majority Leader Steny Hoyer and arranged to sit down with committee chairmen on Friday to go over proposed changes. Officials said the public release of the bill, originally set for Friday, would occur no earlier than Monday.

It was the second setback in three days for President Barack Obama's top domestic priority, although it was unclear whether it would amount to anything more than a brief delay for a bill of enormous complexity and controversy.

There was upheaval earlier in the week in the Senate, where the Democratic leadership is intent on scuttling a proposed tax on health care benefits that has long been key to attempts at a bipartisan compromise. At the same time, Senate Majority Leader Harry Reid and others went out of their way during the day to emphasize eagerness for Republican support.

As an alternative to the benefits tax, Democrats are considering raising taxes on wealthy investors to help pay for health care legislation, along with numerous other options, according to officials who spoke on condition of anonymity. The proposal to extend the current 1.45 percent Medicare payroll tax to capital gains earned by high-income taxpayers would bring in an estimated $100 billion over 10 years.

In the House, Hoyer sought to minimize the day's developments, which occurred as Democrats on one committee were making final decisions on provisions to pay for the legislation.

"Let me make it very clear that everybody in that room thinks we ought to pass health care reform," the Maryland Democrat said.

One conservative Democrat, Rep. Mike Ross, D-Ark., said he believes no House vote should take place until September. That is well past a midsummer informal deadline set by Pelosi, D-Calif.

"I promised the president that we would have legislation out of the House before we went on an August break," Pelosi said earlier in the day. "That is still my goal."

Despite some success _ the nation's hospitals agreed to a cut of $155 billion in projected Medicare and Medicaid payments _ progress has been scant and internal differences magnified.

In general, any bill that emerges from Congress is expected to follow Obama's blueprint for reining in health care costs overall while extending coverage to 50 million who lack it.

Another objective is to make sure that insurance companies can no longer deny coverage or raise premiums to unaffordable levels to individuals with pre-existing medical conditions.

But literally hundreds of details are involved in drafting legislation, and gaining a consensus even among Democrats is proving to be remarkably _ if predictably _ difficult, despite their large majorities in both houses.

As an example, some Democrats are demanding legislation that permits the government to sell insurance in competition with private companies. Republicans overwhelmingly oppose such a plan, deeming it a stalking horse for universal government-run insurance, and many Democrats have concerns, as well.

Some Democrats prefer a plan for a nonprofit cooperative to take the place of government in competing with private companies. Others favor a government role only in cases in which consumers lack a choice in coverage.

Similarly, Democrats are divided on paying for the bill, some preferring more tax increases than others, some favoring more cuts in Medicare and Medicaid.

"We've just got a lot of question and the top of the list would be how to pay for it," said Rep. Marion Berry, D-Ark., one of the Blue Dogs.

"I don't think we have significant cost-containment mechanisms in the proposal yet," said Rep. Adam Schiff, D-Calif. He said he favors provisions aimed at preventing overtreatment of patients and overpayments to doctors, hospitals and other providers.

A dispute over tax increases was at the core of upheaval in the Senate earlier in the week.

Sen. Max Baucus, D-Mont., and chairman of the Finance Committee, has been working for months with Republicans in hopes of gaining support for a bipartisan bill that can command 60 votes.

Efforts to raise money to pay for subsidizing the cost of insurance had focused on a tax on health care benefits for workers with high-cost coverage provided by their employers.

Baucus and Republican supporters argued it would also have tended to reduce the cost of health care overall, as well as offset the cost of the bill. But the Democratic leadership stepped in forcefully, citing poor public polling, opposition of organized labor and concerns about taxing middle-income workers.

There are over 1200 different insurers and tens of thousands of different polices and options. Today, though it might be difficult, you can control your treatment. Under politician controlled medical treatment, no options, no choices, no debate, no getting different treatment than what the bureaucrat in El Paso says, no getting treatment saving your life and fighting it out over the insurance later, options will be as in Canada, France, et al ILLEGAL. If you cost too much under the Obama plan you get pain pills and a hospice.

And politician controlled rationing is exactly what Obama wants, he repeatedly says so himself. Obama bills health care reform as a cost saving, budget-balancing measure that would start to deny payments for treatments deemed "ineffective," or (as one acolyte put it) when "a person's life, or health, is not worth the price."

Politician controlled medical treatment, that costs over a $1,000,000,000,000, places your internal organs directly under the control of bureaucrats in Salt Lake City (or your own local government combined post office and medical treatment review board), institutes rationing and makes it Illegal to pursue any other medical care. This will not be an easy sell. Which is why all the hurry to pass it in secret just like the stimulus and "cap and trade".

How can you people not get it? You are already being rationed by insurance agencies that restrict your access to care by denying claims for payment, or denying a procedure that your doctor feels is necessary while they skim up to 30% of your healthcare dollar to line their pockets.
I am a physician, and believe me, single payer is the best thing that could happen to the U.S. in their effort to cover everyone and decrease costs.

You're making no sense by merely stringing words together. And keep in mind you can't get reliable information from FOX.

Social Security and Medicare are both successful "government-run" programs. Are you willing to give up your benefits from Social Security and Medicare because they're "under the control of bureaucrats?"

We (including you) all pay more for health care because of those who are not covered and can not pay. Those who are not covered and CAN pay (which means they pay out of pocket) have to pay much more. Those who ARE covered probably haven't noticed that the cost for treatment has increased, which in turn results in an increase in premiums. When the premiums become unaffordable for employers, the employees will be dropped from coverage. When the premiums become unaffordable for individuals, they will drop their health insurance to make sure they can pay the rent.

If we don't reform the system now, insurance premiums will continue to rise, more people will be dropped and they will fall into one of the two categories of uninsured Americans. Then the cycle begins again. We need to "break the chain." We need healthcare reform, now! Health insurance from the greed-infested private sector has been a disaster! The federal government must be involved!

Tobin, read more about this bill, billions are going into pork projects like sidewalks and parks, because the promote preventative healthcare­... "healthy living" you think this law will shrink your bills if they putting healthcare dollars into paving projects???

Im not against having beautiful parks and bike paths but why can't that be covered under a different bill unrelated to fixing the healthcare problem.

The people who wrote this 1200 page bill aren't as concerned about affordable healthcare as they are controlling your money. We won't get single-payer plan and not every American will get coverage, but we'll have new streetlights and farmers markets. A lot of place have these things, and in fact the outdoors is EVERYWHERE. its up to people to go out and be active. Terrible logic in this spending.

This is really sad. The more people work, the less they seem to have, and it is not because of taxes. And now the jobs are disappearing. Every single discussion in this country turns into an idealogical fight between the left and the right. We have turned against each other. It is like a bitter divorce where the attorneys for either side win, but the family loses. Skyrocketing health care costs affect everyone. We don't need external enemies, we are collapsing due to this civil war of politics.

Btw, we have been told since the early 90's that no ones job is safe. They why are we supposed to depend on large employers to provide insurance for us? Does anyone think ahead? And really, no matter what political stripe you happen to be, why would you defend the insurance companies right to have no competition? Health insurance is a commodity. What kind of lawmaker puts the needs of a corporation over the wants of his consituents?

WTF is wrong with this country?

Congress is filled with crooks, making bold-faced lies to the American people. Preparing to waste more money in the worst recession in 100 years and at the end of the day healthcare reform will be in name only.

They need to start this healthcare bill from scratch and get it right. The bill they have shouldn't pass, its going to do nothing to make life easier for the common person.

No universal coverage

Health care is not a "want", it's a NEED. All of the people who put coffee cans at the local convenience store with a poignant health care story begging for money, would rather not have had to become beggars. The people who favor the status quo are spreading a lot of money around Washington, and our so-called representatives are more afraid of losing that cash than they are of the anger of their constituents. They're sure that we all have a short attention span and will forget all about it come election time. I think on this issue they couldn't be more wrong. Beware to the pol who sells out the people that he or she is supposed to represent. That's what primaries are for!

-- THIS IS CODE FOR: I'm finding that too many of my Democratic Party caucus members are bought off by lobbyists or are simply Republican-Lite. I don't want Americans to think we are sell-outs so I better start saying we are being bi-partisan and try to fake some moral high ground.

Did you notice last week Reid told his caucus to stop trying to chase Republican votes? That was code for "pass real change". Things are not looking good.

why not investigate more into the bill instead of assume people are voting against it because of lobbyists? Should heathcare reform include park beautification? funding for street lights? THe argument is people will be healthier if they are surrounded by a prettier environment. What a crock... no universal coverage, no single-payer plan, they want to cut insurance premiums, but are taking billions of dollars in this legislation for pork-barrel projects, hardly seems like reform... how is a new sidewalk going to cut healthcare costs? We have a million miles of pavement already, its not preventing illness as they claim it will... congress has a disease, kleptomania, robbing America blind.

No, that's not the argument. Think about it - many people would be much healthier if they would just WALK more. Sidewalks and streetlights provide incentive for people to walk. Construction of these projects means more jobs. If we stimulate the economy by providing jobs for projects that inspire people to get more exercise we are solving two problems simultaneously.

That's not the argument. Those are facts. People are healthier surrounded by green spaces, around certain types of plants, etc. This is not some blue-sky wishful thinking but based on thousands of studies posted in peer-reviewed scientific journals. Even the type of music you listen to affects your health in profound ways.

Blue dogs are rebelling against the proposal, not the idea.

Most moderates voted for Obama to fix the big issues like Health Care, even with a public option and get spending under control in government. Right now he is on the precipice of falling into the 1993 or even worse the 1977 Democratic world of futility.

The cowardly democrats in congress are caving on health care and just want to increase taxes to pay for their special interests and seem to be complete non plusses to the Debt and Tax issues. This is what got Reagan in office and Bush in office. SOP or business as usual is not acceptable to the people.

Here people like MoveOn and Moderates agree. Those far left liberals who call themselves progressive are damaging the entire Obama administration, and fast.

Note to President Obama: Change begins at home.

1. Public option and costs as a percentage of GPP in line with other large countries with better outcomes.
2. There is no greater priority. Health Care costs more (we waste 1 Trillion per year, every year), affects more lives (40 million) than the banks or Iraq.

USA Today reported this week that "counties that supported Obama last year have reaped twice as much money per person from the administration's $787 billion economic stimulus package as those that voted for his Republican rival, Sen. John McCain."

ABC News reported this week that the failed stimulus tracking website run by the White House, Recovery.gov, will get an additional $18 million taxpayer-funded injection to support a "redesign.­" The Washington Examiner's David Freddoso points out that the contract was awarded to a Maryland firm whose donors have contributed $19,000 to Maryland's House Majority Leader Steny Hoyer.

18 million for a website ?

As for it being from a company who has contributed to Hoyer, if that was the ONLY case you'd have a point. Congress is owned by special interests regardless of party .
The answer is Campaign Finance Reform with zero loopholes. Of course that will never happen.

As you will see in the above piece, Washington state business owners are screaming for a public option, but Cantwell is still leaning toward a co-op scheme. I don't know what her problem is given that she is much more liberal generally than Patty Murray (who has already declared for the public option). So email Maria and tell her to get on board!

I share your concerns. I have written and called Senator Cantwell numerous times regarding her position on health care reform and urge her to support true reform, not some patch-work quilt of "compromises" that are really a sell-out to the insurance companies.
The Republicans don't want any health care reform and any attempt to compromise is to simply abandon health care reform altogether.

People need healthcare wheather they have a job or not. We didn't have healthcare last year, and we lost jobs. We haven't had healthare for the last 50 years and jobs came and went. Why would you tie jobs to healtcare. Jobs will come when people have faith in their governments economics. After the last 30 or so years of betrayal of the Middle Class, that trust will be hard earned.

THis healthcare bill own't cover everybody, won't include single-payer and will tax the medical benefits of the middle class... yeah its a great law... BILLIONS of dollars in this fix-the-system bill are going to SIDEWALKS and STREETLAMPS, with the argument that walking around more is preventative healthcare­... this is the logic in the current bill. Its going to cost every taxpayer thousands of dollars in new taxes and it won't even fix the problems.

I'll scream this from the rooftops. THese democrats might be the smart ones, bring on good legislation instead of a healthcare reform in name only. Nobody will be happy with the results... in fact no one voting on it will even read the law from front to back.
