I'm not a doctor (and I don't play one on TV) but I think when you have a pancreas disorder you can't process regular food and have to go on a strict liquid diet. (The pancreas is responsible for secreting a digestive juice which breaks down food.)
Judging by the picture it looks like he hasn't eaten in a while. I guess now he knows what most North Korean citizens feel like on a daily basis.
