A Mexican reporter failed big time on live television when he tumbled into the very large street puddle he was reporting on. This is a particularly bad fail because he was so aware of the puddle he splashed into it. He had to be pulled out by his producers. The cameraman did the right thing, of course, in simply letting the video roll on.
