Young Americans are smoking fewer cigarettes, but researchers say that progress is threatened by an increasingly popular cultural import -- the tall, ornate water pipe known as the hookah.

What started as a fad for smoking exotic and flavor-soaked tobacco in hookah bars is showing worrisome staying power, according to a study released on Tuesday by the Wake Forest Baptist Medical Center in Winston-Salem, North Carolina.

"This may serve as a starter product for other tobacco uses," said Erin L. Sutfin, an assistant professor and lead author of the study.

Sutfin said she and fellow researchers carried out the research because little has been done to assess the prevalence of hookah smoking and what perceptions, or misperceptions, feed its popularity.

"Unfortunately, many young adults are misinformed about the safety of hookah smoking, and some mistakenly believe it to be safer than cigarette smoking," she said.

The study results were drawn from a Web-based survey of a random sample of 3,770 students at eight North Carolina colleges and universities.

The researchers found that 40 percent of the students -- more than one-third of those surveyed -- reported having smoked tobacco from a hookah, while nearly 47 percent reported having smoked a cigarette.

Sutfin said 22 percent of the students who said they were hookah smokers had never smoked a cigarette, indicating that successful efforts to discourage one health threat could be getting undermined by another.

Hookah smokers who don't move on to other products -- such as cigarettes, cigars or smokeless tobacco -- still face health threats from the hookah smoke, Sutfin said.

The smoke, often inhaled more deeply because it is cooled by water and flavorful, contains high levels of toxic compounds, including tar, carbon monoxide, heavy metals and cancer-causing chemicals.

Despite laws in North Carolina and other states that ban smoking in most bars and restaurants, hookah bars generally are not covered because they don't serve food or alcohol or the hookahs are used outside.

Hookah bars are becoming common fixtures in college towns.

At one such bar near the campus of North Carolina State University in Raleigh, two young men on Tuesday smoked from a hookah in an Arabian-style room full of couches and draped fabric.

Both said they were aware of the health risks but felt hookah smoking was not as bad as smoking cigarettes.

"I can't smoke a cigarette to save my life. I just can't. It's too harsh," said Chris Conrad, 26, who smokes a hookah once or twice a week. "I never feel that this is any way addicting or anything."

Tom Battle, 26, said hookah bars are "like a cigar bar for 20-somethings."

"It's not something I plan to do the rest of my life," he said. "But I'm young and once in awhile I'll have a hookah."

I smoked at hookah bars all the time in college. It's been 3 years since I graduated, and I've only been back to a hookah bar once. It hasn't made me into a cigarette smoker. It's a fun, easy way to go out with friends that's an alternative to eating out or drinking. I'm not saying it's healthy, but neither is overeating or booze.

Hookahs are much like bongs, and therefore suitable to smoke combustibles like medical marijuana- if it were legal in my state, and it is not.

You can't carry hookah with you everywhere, you can't pop out on a work break, or in between classes for hookah, etc. etc. Given the relative inconvenience of hookah, I'd be surprised to find it's worse than cigarettes, or even close, in the long term.

Here come more laws, more controls, more violence used against peaceful people.

And don't give me this,"it costs us in healthcare" garbage. Once your money has been stolen from you it is gone. Stop worrying about what the thieves do with your money after they steal it. Focus on stopping the theft. You do not pay takes because it is your duty. That is just propaganda you have been fed since your first day in the government schools you were forced to attend. You pay taxes because if you don't the IRS will make your life HeII! Go ahead and try not paying. They will steal your property, garnish your wages and/or chain and cage you like a rabid animal. It is VIOLENCE. Pay or you will PAY!

Smoking cigarettes, hookha or marijuana are all equally bad for you. There are plenty of reasons to make marijuana legal, so let's lay off the "marijuana produces magical smoke that doesn't have negative effects on your health" argument. Yes, it's less addictive and yes it's less convenient (mainly because it's illegal) but smoking X amount of marijuana has the same negative health consequences as smoking X amount of tobacco.

A more apt comparison is marijauna and alcohol.

The best way to use medical marijuana is to eat it, and avoid any exposure to smoke. The tar in pot is similar to the tar in tobacco but smokers inhale deeper and hold it longer but I don't know any pot smoker that smokes 20 a day. Pot also does not contain nicotine, the addictive substance in tobacco. A physical withdrawal syndrome with pot has only been seen in heavy users (75 joints per week over 20 years). We are just now seeing the health effects from long term pot use, which is COPD caused by the tar.

First of all, tobacco smoke is tobacco smoke. Avoid it like the plague it is. Sure, when you are young it seems glamorous, but wait until you are a bit older, and vital in most ways - yes still horny, but you are out of breath. Your chest hurts. Soon you have no stamina. Its freaking over dude. Before you know it it feels like a linebacker is standing on your thorax.

I am earning a living in the Middle East. Everyone smokes. Hell, the babies smoke. You can't go anywhere without breathing this sh*t. Believe me, I would not be here if I didn't have to.

Try to remember a couple things about the way cool middle east. No alcohol. No women. No nothing. Its all smoke and sweets. Really, we can all stop worrying about so-called Islamic terror. These people will all go down in diabetic shock and coughing fits at an early age.

Do your selves a favor. Smoke a little weed in a small pipe on occasion, but the stay the hell away from tobacco.

I smoked ciggarettes for awhile then when we got a Hookah in the house..I smoked nothing other...only after they were all gone had I realized how additcted I had become! The hookah inhales such a huge amout per smoke that you hardly realize how much your taking in....that becomes an addiction.
