Obama Publicly Backs Means-Testing Medicare (UPDATE)

WASHINGTON -- President Barack Obama formally acknowledged on Friday that he would support a plan to means-test Medicare as a part of a deal to raise the nation’s debt ceiling.

“I have said that means-testing on Medicare, meaning people like myself -- I’m going to be turning 50 in a week, so I’m starting to think a little bit more about Medicare eligibility -- but you can envision a situation for somebody in my position, me having to pay a little bit more on premiums or co-pays would be appropriate. And again, that would make a difference,” the president said at a press conference. “What we are not willing to do is restructure the program in the ways we have seen coming out of the House in recent months.”

The comment was the first public acknowledgment from the White House that the president would support changing the payment structure of the entitlement program. Prior to Obama’s remarks, multiple sources in both parties told The Huffington Post that the administration was making it clear to debt ceiling negotiators that such a structural change to Medicare was on the table.

The proposal is not entirely controversial among health care economists. But it will rankle a good chunk of the president's own party, which has sought to keep Medicare's structure as a basic insurance program. Medicare premiums for doctors and for prescription drugs are already means tested. Making top earners pay even more -- while potentially sound policy -- opens the program to the politically potent charge that it is becoming health care welfare for lower income Americans.

The Obama administration's embrace of the idea came during talks between lawmakers and Vice President Joseph Biden. The exact contours of what was proposed are not entirely clear. But a version that House Majority Leader Eric Cantor (R-Va.) proposed in later discussions would have saved the government an estimated $38 billion by charging those high-income beneficiaries 10 percent more for the cost of hospital stays and prescription drugs.

Obama's nominal support for means-testing Medicare, however, does fit into the larger outlines of his plan for the debt ceiling debate. In an effort to both win the support of Republicans and tackle as many deficit-contributing issues as possible, the administration has placed entitlement programs like Social Security and Medicare ("sacred cows" for the Democratic Party) squarely on the table. The president also lent his support to a plan to raise the eligibility age of Medicare from 65 to 67, over the course of roughly 25 years. His team has, additionally, discussed various changes to the way in which Social Security benefits are measured and paid.

Both of those ideas encountered immediate push back within Democratic and even moderate Republican circles. Means-testing Medicare has more political support. Rep. James Clyburn (D-S.C.) a top-ranking Democrat in the caucus, has embraced the idea, as have the party's more fiscally conservative members. In the past, the administration too has embraced the idea. The president's own health care law placed a small tax increase on the wealthiest Americans, a fee that could, theoretically, be considered a form of means-testing. And in his 2009 budget, the Obama administration suggested means-testing the prescription drug portion of Medicare.

A request for comment for this article was not immediately returned by an administration aide.

UPDATE : 1:30 p.m. -- An administration official sent HuffPost the following statement in order to point out that President Obama was not advocating lowering the income threshold at which Medicare beneficiaries would be asked to pay higher premiums.

Currently, Medicare premiums for doctors and for prescription drugs are already means tested, meaning that couples making over $170,000 or singles over $85,000 (about 5% of Medicare beneficiaries) already pay somewhat higher Medicare premiums.

What the President referenced today was his openness, as part of a potential big deal, to asking Medicare recipients over those high-income thresholds to pay modestly higher premiums. At no point did the Administration express openness to raising premiums on Medicare beneficiaries below those income levels.

(Note that the Affordable Care Act already included a nominal freeze of those high income thresholds through 2019, resulting in modestly greater means-testing for high income Medicare beneficiaries).

Please work to help everyone understand the difference between "means testing" and income related deductibles & co-insurance. Medicaid, the healthcare program for people with low income is a "means tested" program ... (one has to have income below a certain level to receive the benefits). Medicare is a social insurance program .. everyone contributes and participates regardless of "means". However, because of our current financial issues ... some of the components of the Medicare program have "income-related" premiums, co-insurance & deductibles .. so that those that have more resources pay more... that is not a Means test!

For all the world to see...I will give you MHO as to government economics, which is -completely- different from corporate economics.

EVERY DOLLAR THAT THE GOVERNMENT SPENDS SHOULD BE "MEANS" TESTED!

Corporate economics is "all" about profits. That is fundamental, and correct, and there is no use arguing that fact.

Government economics is "all" about peace and prosperity. That is fundamental, and correct, and there is no use arguing that fact.

Where there is "synergy" between profits and peace&prosperity the nation will thrive. There is absolutely NO such synergy in America today...no matter how much the Gates Foundation and others try to do to correct the balance.

The rallying cry for 'more' profits is surely the destruction of any government economic authority, and if that's the case, THIS America will never again find peace and prosperity. It cannot be done with profits alone...and that is why trickle-down theory is a pathway to the destruction of the US government.

I didn't make it that way, and it really isn't my opinion....just simply the TRUTH.

Your right, but how to get the truth past all the media noise is the challenge. The American people are so disgusted with their own government that right now 'most' would just cheer if DC burned to the ground again, and sadly that is just the attitude that these right-wingers have been promoting.

When everything is "entertainment", nothing matters; reverse Stockholm syndrome...and sadly the situation has progressed to the point where only shock therapy will suffice. A bigger shock than 911, because remember Bush's battle cry, "Keep spending!". Classic psychological misdirection because "there are profits in chaos"....and note the resultant destruction of peace&prosperity.

When President Obama retires he gets a sizable federal pension and basically free medical care. Since his retirement is set, and he knows he'll be taken care of, and he doesn't need Social Security or Medicare .... unlike the millions of Americans who depend on these programs during their retirement .... it's becomes much easier for him to bargain them away.

many of obamaphiles take offense when i say that obama wants to kill medicare and social security. and on the face of it Obama is merely cutting it so far, but by means testing social security he turns these programs into the charity cases along the lines of welfare and medicaid.
we already ended welfare as we knew it... . and go find a private doctor who accepts medicaid ... .
if means tested medicare will die a long and agonizing death while the levels of care under it will continuously deteriorate. social security is vulnerable now but if means tested it would be defenseless. This is when i say that Obama wants to kill Social Security and Medicare I do not exaggerate.

Raising premiums on the Medicare and Social Security INSURANCE programs for the wealthy will not kill the programs. In fact, offering Medicare to everyone who wants it at premium rates set at a reasonable % of yearly income would fix it forever. You really do have to broaden your education a bit. Try something besides Faux Snooze. Congratulations...that's 2 comments so far. Do you get paid 50 cents or a dollar apiece?

Is it true? There is a long wait for medical appointments in Canada? That's the argument anti-Affordable Care Act people use. They don't mention that there are places in US where women have to make mammogram appointments a year in advance, specialists months in advance, because that will hurt their argument. So what's the difference in access in Canada?

I vacation with lots of Canadians, and I haven't heard even one of them make those kinds of complaints. In fact, they feel sorry for us! They can't understand what's wrong with us for allowing all the corruption. Fired up, take a vacation in Canada. You won't have to worry about getting sick.
My husband actually had to go to a hospital in Canada, and we understood there was no bill (hospital). But then, as we were leaving, a young doctor was chasing down the hall after us...saying we hadn't paid his bill yet...$16.! This was some time ago, but I bet it would still be very reasonable.

Thats the same kind of experience i had when i mentioned how bad the british healthcare system was with their long waits and flat out denial of care to a group of brits at a convention i was at . You should of heard the laughter . Then one of them said they wouldn' t trade their healthcare delivery system for ours and speaking quite frankly they felt sorry for the average american if he got seriously ill not knowing who will pay for what ! and how much that asperin cost or who would pay for it and when.
