Health Care Exchanges: National Governors Association Advises Obama Administration

The nation's governors sent the Obama Administration recommendations Thursday morning regarding the implementation of health care exchanges as a federal-state partnership.

The National Governors Association wrote a letter to Health and Human Services Secretary Kathleen Sebelius full of proposals that they say would allow for the states to work with the federal government to set up jointly managed exchanges, while at the same time retaining autonomy for the states in future decisions. The letter said that many of the states which are considering the partnership model are doing so for the short term benefits of having federal management while states get programs up and running or are compelled by the option to divide up duties between states and the federal government. The letter was sent by NGA Health and Human Services Committee Chairman Iowa Gov. Terry Branstad (R) and Vice Chairman Illinois Gov. Pat Quinn (D).

HHS proposed the partnership option to states in September as a way to help get the programs off the ground. As part of the federal health care reform law, the exchanges need to be in place by 2014.

One of the concerns addressed by the governors was the investments states have made in other health care programs, which the NGA feels the federal government would be taking over as part of the partnership proposed by Sebelius.

"Under the proposed partnership models, states would be required to cede many operations that have been traditionally handled at the state level -- such as Medicaid eligibility -- regardless of whether they implement a state-based exchange, implement a state-federal partnership model or turn over responsibility for the exchange entirely to the federal government," the NGA wrote. "States have invested taxpayer resources in state-based eligibility systems since the Medicaid program began and want to avoid duplication of effort."

Branstad and Quinn also wrote that state governments are concerned that entering into a partnership would prevent states from being able to take full control of the state exchanges at a future date. The governors wrote that states are still undecided on a final course of action due to uncertainty over final rules and funding.

Among the recommendations from Branstad and Quinn are to have states formally assign functions to the federal government that have been handled primarily on the federal level.

"To best facilitate a state-federal partnership, we suggest the federal government offer states the option of assigning functions where states have little or no current operational role such as: determination of eligibility and appeals for federal tax credits and subsidies; facilitation of advance payment of premium tax credits by the Department of the Treasury to insurers; enforcement of the individual responsibility provisions; and reporting of user and employer data to the Department of the Treasury," the governors wrote.

Branstad and Quinn also proposed having the federal government work with the states in the partnerships to provide assistance on information technology issues, including software development.

The letter comes as state governments continue to grapple with the decisions over setting up the exchanges, including over a dozen Republican-led states rejecting the concept. Ohio voters are voting next week on a constitutional amendment to block the federal health law from being implemented in the state, and North Dakota lawmakers are going into special session next week to tackle a host of issues, including health care exchanges.

Obama's health care plan is SOCIALIZED MEDICINE! Nothing more, nothing less. You do not want this, believe me. I have seen first hand, the results of being told where and to whom you may go for health care. And if you think for one minute you can go to the best Doctor for your treatment, you are sadly mistaken. You will be told where and to whom you may go. You will be denied treatment based on the cost of it. And if you are a senior- forget it. Health care will be null and void for you. After all, Obama thinks seniors are a disposal commodity. Very much like Nazi Germany. There is a reason everyone in the world seeks health care in the United States, because right now it is the best in the world. A health care package such as this, can not and should not be put together just for the sake of politics of a party. It takes time and a lot of research to establish one. Neither of which were done. It was pushed through just so the democrats could say they passed HealthCare Reform. You do not want this health care- think about it very carefully for your life may very well depend on it.

My doctor took from me everything he could get away with.

I say he had to give me a patient education to fill me in of his planes.

I am still baffled by the fact that I require constant monitoring and care from the US Government. What did people do before "The Government" provided everything from cradle to grave?

Get the Federal Goverment the ELL outta my life, go back to DC collect taxes and raise an army for the common defense! Thats it nothing more!

Why will I be the first to want help from the FEDS??? What can they do for me, give me a FEMA card hold my hand and then dissappear? Get a grip it is your community and friends who help you, not some Washington Double Dealing slime!

I am pleased to see reporting on the real complicating process involved in the implement of such new policies for the greater of the population, Understanding that it is not simple but there is a plan and it is being handled increases the credibility of the plan and the acceptance from the public for the benefitis and positive overall impact.

It also makes the neg pushback from noncompiant states appears to be small minded and
simplier in its resistant messaging and actions.

Feel free to plagiarize, politicians do it and I urge you to use your imagination, Keep Women's Rights, Repeal Republicans, it seems like an endless fountain of yard signs and the Republicans keep the water flowing. How can I monetize this?

WHY? I do not believe you know the extent of this horrible Law your Mr. Obama made on over 2000 pages. Have you read the entire law and DO YOU understand most of it? IF you do, you do not want this because it will control every part of your life. You will not be able to say WHAT doctor you want, what hospital you want, what medicine you NEED OR surgery you may need. If you are in the over 60 group, you will not be given necessary medical care IF (someone in Congress) says you are not worth the cost of Medical Care, so sorry, just take an aspirin and don't bother calling in the morning.. I really would like an answer to WHY?

I'm a patient man, if the world becomes unbearable due to this law, I'll join your revolution and we'll take the monsters down, otherwise, get back to me in a couple of years and we'll see if you feel the same and if your dire warnings have come true. this plan would be very good for states, small businesses and individuals that need insurance. why most states are fighting this is beyond me. currently if you don't have a job, there is no way for you to purchase insurance excerpt you apply for state aid. this new exchange would reduce the burden on state, allow people to shop for healthcare like auto insurance. it would eventually bring down the cost of healthcare premium because of competition. we also have to allow insurance companies from other state to sell their product in different states.
