Truvada: FDA Favors First Drug For HIV Prevention

WASHINGTON — A pill that has long been used to treat HIV has moved one step closer to becoming the first drug approved to prevent healthy people from becoming infected with the virus that causes AIDS.

The Food and Drug Administration said Tuesday that Gilead Sciences' Truvada appears to be safe and effective for HIV prevention. It concluded that taking the pill daily could spare patients "infection with a serious and life-threatening illness that requires lifelong treatment."

On Thursday a panel of FDA advisers will consider the review when it votes on whether Truvada should be approved as a preventative treatment for people who are at high risk of contracting HIV through sexual intercourse. The FDA is not required to follow the advice of its panels, but it usually does.

An estimated 1.2 million Americans have HIV, which attacks the immune system and, unless treated with antiviral drugs, develops into AIDS, a fatal condition in which the body cannot fight off infections. If Truvada is approved, it would be a major breakthrough in the 30-year campaign against the AIDS epidemic. There have been no other drugs proven to prevent HIV and a vaccine is believed to be decades away.

Gilead Sciences Inc., based in Foster City, Calif., has marketed Truvada since 2004 as a treatment for people who are infected with the virus. The medication is a combination of two older HIV drugs, Emtriva and Viread. Doctors usually prescribe the medications as part of a drug cocktail that makes it harder for the virus to reproduce. Patients with low viral levels have reduced symptoms and are far less likely to develop AIDS.

Researchers first reported that Truvada could prevent people from contracting HIV in 2010. A three-year study found that daily doses cut the risk of infection in healthy gay and bisexual men by 44 percent, when accompanied by condoms and counseling. Another study found that Truvada reduced infection by 75 percent in heterosexual couples in which one partner was infected with HIV and the other was not.

Because Truvada is already on the market to manage HIV, some doctors already prescribe it as a preventive measure. But FDA approval would allow the drugmaker Gilead Sciences to formally market its drug for that use.

FDA reviewers on Tuesday said that patients must be diligent about taking the pill every day if using it as a preventative measure. Adherence to the medication was less than perfect in clinical trials, and reviewers said that patients in the real world may forget to take their medication even more than those in clinical studies.

Some patient advocacy groups say the drug is an important new option to prevent HIV, alongside condoms, counseling and other measures. Last month, advocacy group AIDS United and more than a dozen other groups sent a letter to the FDA, urging approval of Truvada.

"If we're going to reduce the more than 50,000 new HIV infections in this country each year, we need to increase the available options for people," said Ronald Johnson, AIDS United's vice president.

But support for FDA approval is not unanimous.

Although the FDA is legally barred from considering cost when reviewing drugs, health care providers have raised concerns about Truvada's price tag: $900 a month, or just under $11,000 per year. Medicare and Medicaid, the nation's largest health insurance plans, generally cover drugs approved by the FDA, and analysts expect most large health insurers to follow suit.

Additionally, some researchers say the prevention pill is not the chemical equivalent of condoms, which they say remain the best weapon against AIDS. They also worry about Truvada's mixed success rate in preventing infection among women: Last year, a study in women was stopped early after researchers found that women taking the drug were more likely to become infected than those taking placebo.

Researchers speculated that women may require a higher dose of the drug to prevent infection. They also said the disappointing results may have resulted from women not taking the pills consistently.

"We know that if the person doesn't take the medication every day they will not be protected," said Dr. Rodney Wright, director of HIV programs at Montefiore Medical Center in New York and chairman of the AIDS Health Foundation. "So the concern is that there may not be adequate adherence to provide protection in the general population."

No. What it does is remind us that our health care system is a joke. The drug is indicated for daily preventive dosing by those "at risk," who are not infected with HIV. What is that group? Young gay men, especially of color, and IV drug abusers. How many such people have health insurance? If not, how many can afford $14,000 per year to prevent something that hasn't happened--and may never happen? It's just another reminder to these people that they're on the outside looking in on a system only the Romney family can afford.

Truvada is a drug combination that costs $9.00 per month as a generic overseas and $750 per month via Canadian pharmacies. In the USA it's only a "breakthrough" for Gilead Pharmaceuticals' bottom line. Forget the hopeless craziness of the Republicans, who consistently vote against themselves. It's about time that we do three things: (1) Universal care, i.e., Medicare for All. (2) Regulate the obscene profits of the parasitic pharmaceutical industry. (3) End the idiotic War on Drugs that keeps IV drug abusers beyond the reach of public health policy.

Introducing this pill as a form of "protection" against HIV is COMPLETELY IRRESPONSIBLE. This is like the morning after pill, except that one has to take it continuously. TRUVADA CAUSES KIDNEY PROBLEMS AND FAILURE IN MANY PEOPLE!!!!! It is a very toxic drug and to market it to the masses as an alternative to condoms is REPREHENSIBLE. This drug changes and rewrites your cellular t-cell rna/dna codes. In other words it changes you body composition. The one pill a day will cost over $2,200.00 dollars a month!! If you take two pills a day it will cost $4,400.00 a month!!! This is NOT the path to go down, but I am sure that Gilead and people at the FDA want to make BIG money off of it at YOUR expense.

This is just one more step in finding a cure. Some day they probably will find cures for many diseases while testing experimental drugs intended for other uses. Good work on this, keep the research.

As someone living with HIV for 27 years, I RESPECTFULLY DISAGREE WITH YOU. This is not a step towards a cure! Not medically or scientifically. This pill rewrites the T cell rna/dna code and changes your body. It causes KIDNEY FAILURE in more than a few percent of people.

So, you're ok with the $4,400.00 dollar a month cost? Are you ok with the fact that this pill takes the T CELL (white blood celll that fights infection) and rewrites its dna/rna codes and changes your body. Over the long term it causes kidney failure in many people. Everybody wants a quick easy way to continue rampant destructive sexual lifestyles. It is NOT judgmental it is prudent.

How does counseling help reduce the risk of any kind of infection? Maybe I should seek counceling to help reduce my risk of becming infected with the common cold.

because counseling targets people's risky behaviour - getting them to reduce hi-risk activity thereby reducing risk - u dolt....

regarding the common cold -- wash hands frequently, drink plenty of water, don't share drinks, utensils, bodily fluids, , etc with someone with a cold --

At the cost of $4,400.00 dollars a month!!! It has to be used with condoms anyway!! Most people are going to take the pill and skip the condoms. This is a disaster waiting to happen.

As it seems to have been around a long time, and already proven efficacious, I see no reason why it shouldn't be prescribed as a preventive (along with condoms, as mentioned.)

What I am wondering about is why the testing was stopped so quickly on women. They didn't even go far enough to find out if women just needed a higher dose.

I lost my best friend to AIDS because her husband had the illness (he wasn't gay, he had been a drug user), back in the 80's. If Truvada could possibly prevent other women in similar situations from losing their lives as well, I am definitely for it. Not to mention the obvious health benefits to the gay community.

It is only 44% effective when used with a condom. Most people will take the pill and skip the condom!! It costs $4,400.00 dollars a month for the pill and it cause kidney failure in many people. Irreversible kidney failure, but you go ahead.

I agree that there is bound to be misuse, but that is true of any drug. I was merely saying I did not see any reason for the FDA to not approve it for that use.

That's a sweet idea, and I wish it were that easy, but it isn't. What if your true love turns out to have HIV and you didn't know it? Or maybe HE didn't know it, and you both find out later. Wouldn't you want something like this to be available?

There is a way to prevent HIV. Safe intercourse and safe needle use. People who cannot do these simple steps are not going to take a preventative medicine as it won't happen to them. And cost will make this prohibitive.

MY GOD! Has the world gone completely mad? If you are screwing around, having sex in a risky manner, have you ever thought about changing the way that you get off? I am disgusted that the Federal Government would be spending this much money, time and effort on such a stupid idea. There is nothing in the Constitution that guarantees you that you can continue to have sex in any manner, and that the government will protect you, care for you, and make sure that you don't catch a disease!

I have been on it for 9 years and the cost has steadily risen over this time! It costs $4,400.00 per month!!!! That is $52,800.00 dollars a year!!!!!!

This is not a solution, this is simply a means for the pharmaceutical companies to make more money off this already astronomically priced medication! Wake up people, the best means to prevent an illness of this nature is education....not popping a pill and crossing your fingers!

it's not THE solution, but it certainly is A solution. education can't prevent everything. even if you know everything about aids, are you going to ask your partner to get tested every single time before having sex? i don't think so.

I'm not saying education is the end all fix, though I would think you would agree it a valuable option in regards to effective preventative measures regarding any disease. There are many people whom will just take the word of big pharma and take these thinking they are full proof.....all they are is expensive and not even nominally preventative as they don't even reach the 50% threshold.
