Whooping Cough Epidemic Declared In Washington State

SEATTLE -- Washington state's worst outbreak of whooping cough in decades has prompted health officials to declare an epidemic, seek help from federal experts and urge residents to get vaccinated amid worry that cases of the highly contagious disease could spike much higher.

It's the first state to declare a whooping cough, or pertussis, epidemic since 2010, when California had more than 9,000 cases, including 10 deaths. Washington has had 10 times the cases reported in 2011, and so has Wisconsin with nearly 2,000 cases this year, though that state has not declared an epidemic.

California responded to its crisis two years ago with a public information campaign, readily available vaccines and a new law requiring a booster shot for middle- and high-school students. Doctors were urged to spot whooping cough early, send infected babies to the hospital and promptly treat those diagnosed. In 2011, the number of cases there dropped significantly.

In Washington, about 1,280 cases have been reported in 2012, and officials believe the state could see as many as 3,000 cases by year's end. Health Secretary Mary Selecky declared the epidemic April 3, and since then officials have bought up the vaccine and made it available for free for people who don't have insurance.

State officials have asked hospitals to vaccinate every adult who goes home with a new baby, and urged businesses to encourage their employees to get the adult booster shot. Washington already requires a booster shot for middle- and high-school students.

Last week, Gov. Chris Gregoire announced the state is putting $90,000 into a public awareness campaign and diverting some federal money to pay for 27,000 doses of vaccine. The state has also asked the Centers for Disease Control and Prevention to send a special team of investigators and an epidemiologist to the Washington.

State epidemic declarations are up to the states; there are no federal regulations for such decisions. Selecky said this is the first time in her 13 years on the job she has declared a state epidemic, but felt she needed to take action to stop the disease from spreading further.

"When we've looked historically, we've seen nothing like this," she said. "We're taking this very seriously."

Adults and teens need booster shots so they don't give pertussis to the babies in their lives, said CDC spokeswoman Alison Patti

"We want to create a cocoon of protection around them," she said. "We're really worried about keeping babies safe."

Pertussis is known as whooping cough because of the "whooping" sound people often make while gasping for air after a coughing fit. A highly contagious bacterial disease, it starts off like a cold but leads to severe coughing that can last for weeks. In rare cases, it can be fatal.

Until routine child vaccination became widespread in the 1940s, pertussis caused thousands of fatalities each year in the United States. While deaths are uncommon today, they still occur: In recent weeks, infants in New Mexico and Idaho have died from the disease.

Because the adult booster for pertussis – called Tdap for tetanus, diphtheria and acellular pertussis – has only been available since 2005, fewer than one in 10 adults have gotten the shot and most don't even know they need it. The numbers are better for teens: about 70 percent have received a booster shot. Most people do not find out they even need a booster until they go to the doctor for a tetanus shot, Patti said.

Patti emphasized that pertussis isn't spreading because of an anti-vaccine movement. Among possible reasons for the recent spike are that diagnoses in teens and adults are getting better and doctors are doing a better job with reporting, she added

Health officials say the disease tends to return in three-to-five-year cycles.

"The incidents tend to oscillate," said Herbert Hethcote, a professor emeritus from the University of Iowa who is a specialist in mathematical modeling of the spread of infectious diseases,

He said the growth of pertussis in Washington state has followed a pattern: As the population ages, the immunity level goes down because the vaccine is wearing off. The disease spreads and the cases grow until more people get the vaccine and the numbers go down again.

Hethcote said his daughter had whooping cough as an adult during the last spike of cases in Washington, four or five years ago. She coughed so hard she broke a rib and was sick for more than a month, he recalls. She never found out how she caught it but the experience raised awareness in his family that pertussis is not just a children's disease.

If only it were that simple. What about older people whose immune systems are down? And what about your coworker who has an infant at home and you infect him or her? What about your son or daughter in school who sit right next to someone who is infected and go home to their family with an infant...and each case infects (without intervention) as many as 15 people? Suddenly and I do mean suddenly, just take a look at the spikes in those states with outbreaks it's here.

To all who blame the non-vaxers: Are you a smoker, are you overweight, do you exercise, do you eat you daily recommended amount of fruits and veggies, do you wash your hands the full 20 seconds? Have you done all you can to make yourselves healthy?

The vaccines are only stated to be about 80 percent effective. If it is the kids not being vaccinated, why are more children getting wc than adults who have not had boosters on years?

Stop blaming the victims. The factors you have listed are nearly irrelevant. None of those factors mitigate against the virulence of a case of Pertussis. It is highly contagious and airborne. Hand washing will have minimal effect on it's transmission. When you crunch the numbers you are 23 times less likely to get Pertusiss if you are current on your vaccinations. Why is that a problem to you?

I am not blamming the victims. I am talking to the people who insist that it is the non-vaxers who are causing the problem. I am just saying that people think just because they get the vaccine, means they will not get the disease. There are giudelines that the AMA reccomends for staying healthy, vaccines are only part of that. Keeping yourself healthy is relevant. If you are 23 times less likely to get Pertussis is you are currnet on your vaccinations, then why are 80 percent of the people who got Pertussis the ones that were vaccinated? Why would we not see a higher percent of the non-vaxers getting pertussis? I am sure there are people exposed to Pertussis that don't get it and who have not had the vaccine, maybe they are just healthy, maybe they eat better, exercise and don't smoke.
The Pertussis vaccine does not seem to be working.

HIGHLY contagious. People are walking around with it and do not even know they have it and they pass it on.

Does this actually make sense to anyone? Let's think about it for a moment. "Patti" states that not getting vaccinated is not a reason for the rise in whooping cough. The alternative explanation for the rise in whooping cough is improved diagnoses, which apparently improved so much over the past year that it accounts for the 10 times increase in cases. Absurd. The New York Times states that last year Washington State "had the highest percentage of parents in the nation who voluntarily exempted their children from one or more vaccines." And the State Secretary of Health admits this could "be a factor in compounding the outbreak." Well duh. Of course, what else would one expect but completely blind denial from the incredibly irresponsible and anti-social parents who refuse to vaccinate?

Well, she is from the CDC, so she may know what she's talking about in this particular case. But surely all those anti-vax folks in Washington can't be helping matters. We're reaching dangerously low immunization levels in many pockets. Measles seems to be especially worrying at the moment.

The rate at which WC is being diagnosed is very alarming. I live on Long Island and the media is always reporting on new cases. Some of the media speculate that the NYC/ NYC metro area has been hard hit because of the visitors and imigrants bringing it here. Others in the media speculate that it has to do with the anti-vaccine movement. Whatever the reason, it can be a fatal disease and it needs to be addressed.

The best thing individuals can do to protect themselves is get the Tdap as an adult and get the Dtap for our babies.

Unless you have had "natural" pertussis during the last few years you are just as susceptible to contracting another infection with the B.pertussis bacterium:

Waning of vaccine-induced immunity has been cited as one of the reasons for the observed epidemiologic trend. A review of the published data on duration of immunity reveals estimates that infection-acquired immunity against pertussis disease wanes after 4-20 years and protective immunity after vaccination wanes after 4-12 years. Further research into the rate of waning of vaccine-acquired immunity will help determine the optimal timing and frequency of booster immunizations and their role in pertussis control.

That vaccine-induced herd immunity is mostly myth can be proven quite simply. In the early days we, we were taught that all of the childhood vaccines lasted a lifetime. This thinking existed for over 70 years. It was not until relatively recently that it was discovered that most of these vaccines lost their effectiveness 2 to 10 years after being given. What this means is that at least half the population, that is the baby boomers, have had no vaccine-induced immunity against any of these diseases for which they had been vaccinated very early in life. In essence, at least 50% or more of the population was unprotected for decades. If we listen to present-day wisdom, we are all at risk of resurgent massive epidemics should the vaccination rate fall below 95%. Yet, we have all lived for at least 30 to 40 years with 50% or less of the population having vaccine protection. That is, herd immunity has not existed in this country for many decades and no resurgent epidemics have occurred. Vaccine-induced herd immunity is a lie used to frighten doctors, public-health officials, other medical personnel, and the public into accepting vaccinations.

There is and never was such a thing as "Herd" immunity. There is only infections and no infections. In immune systems that are dumbed down by vaccine that turn off our TLR's to not recognize them after folding in prion proteins give us by using cells from a woman who had Syhilis,HepB(whatever they thought that was),cancer knowing that the God LIke protein spirochetes carry they have infected us all...But they didn't stop there..Oh no...They were not working as well as intended, so they added even more by injecting Borrelia/Lyme in our feed animals, 1.4mil. people, and our pets, and then declared withdrawn due to lack of sales sold to the highest bidders to continue their crimes against humanity. Because they knew in 1985 that there was no known way to kill the prion synergy created from those proteins. Vaccine made from HeLa cells will be withdraw---- but it is too late.

The CDC has issued specific recommendations about the Tdap booster vaccine to protect infants under one-year-of-age:

Ideally, a pregnant woman should receive a one time booster Tdap Vaccine sometime after 20 weeks gestation to build up immunity against the pertussis bacterium. Alternatively, the booster vaccine can be given to the woman immediately postpartum.

Cocooning-Adolescents and adults (parents,siblings-grandparents, child-care providers who have contact or who expect to have contact with an infant under one-year-old, should receive a one-time Tdap booster vaccine. Ideally these adolescents and adults should be given the Tdap vaccine at least 2 weeks before beginning close contact with the infant.

It is entirely possible that Mr or Ms Lee is God cannot be vaccinated, because of their chronic illness or (perhaps) other considerations. People in these circumstances rely on the rest of us to do the right thing so that diseases such as this don't spread.

It is obvious that many commentators on this article are simply using it as springboard to address some other topic. The article explicitly states that the whooping cough epidemic in Washington has nothing to do with the anti-vaccine movement. Also, nowhere does this article suggest that immigrants are responsible for the epidemic. Instead, health experts posit that better diagnoses, better reporting, or an aging population with lowered immunity due to vaccines wearing off may be to blame.

Rational people actually read the article, put aside preconceived ideas, listen to what the experts have to say, and adjust their understanding accordingly. It's amazing how many people use the news, not to learn something, but merely to bolster their ignorance and prejudices.
