No Obese Child Is an Island

Mary Ann Liebert Publishing recently offered me the position of Editor-in-Chief for the journal, Childhood Obesity, an honor I accepted with great enthusiasm. The topic is timely, compelling, of urgent importance, ever more prominently in the public eye and a particular focus of my work. The journal is off to a great start, is in highly capable hands, and provides an exceptional forum for the combination of scholarly research articles, and pragmatic advocacy for needed changes in policies, practices, and programs. I consider the challenges of this new position a privilege.

But I do have one misgiving, related to the journal's title. For it is long established that no man is an island, and we may readily infer that no woman or child is an island, either. In my view, children and adults will control their weight and get to health together, or almost certainly will not do so at all.

That misgiving was compounded this past week by virtue of my role at the annual Art and Science of Health Promotion conference, held in Colorado Springs. I was asked to give the closing keynote address at the conference, and in doing so, to sum up the "lessons learned" at the conference. This, in turn, required that I attend as many of the conference sessions as I possibly could -- including catching enough of several concurrent sessions during any given block to discern the gist of each.

The conference was excellent overall, and the caliber of most sessions I attended was high. Many sessions -- even most -- addressed the obesity epidemic directly, and most of the rest made at least passing reference to it.

So it was clear enough that the obesity epidemic, and attendant chronic disease, were a major focus of the health promotion community -- as well they should be. It was also clear though, that the target of obesity control efforts was often children -- as often adults -- but rarely if ever ... both.

Experts in worksite wellness were an especially strong contingent at the conference -- no doubt testimony to the growing concern among employers that they are routinely spending more money on health care than on the materials needed to manufacture whatever their company makes. Many insights and innovations related to everything from architecture to incentives were presented, but the one message I never heard was: do something about the kids!

As a parent of five -- as someone who once was a kid -- and as an Internist who takes care of adults but routinely talks to them about what goes on in their homes -- I know something about the power the little devils exert on a family.

When kids have no interest in eating well, but instead want 'the one with Sponge Bob on it' it becomes virtually impossible for parents to make their home a safe nutritional environment. Conversely, when children are motivated to defend a health related issue, they become powerful agents of positive change. Kids were the major drivers of seat belt use, and fire safety nation-wide. They played a key role in reducing tobacco use as well. My own interest in nutrition as a child converted my whole family. For good, or for bad, kids are powerful change agents.

I attended some sessions and reviewed some posters at the conference that were dedicated to the issue of childhood obesity -- and these generally made no particular mention of the parents. In my own public speaking, I am usually invited to talk about adults, or children, but not both (although I do so anyway). Reviews of the obesity prevention and control literature suggest a similar fault line running through families, with children to one side, adults to the other.

To be sure, there are some obesity researchers focused preferentially on families; my friend and respected colleague, Dr. Len Epstein at SUNY Buffalo, springs immediately to mind. But this work is the exception rather than the rule, and still tends to be unidirectional in its intent. Dr. Epstein is best known as a childhood obesity researcher, who involves parents principally to facilitate weight control in the kids.

To some extent, a preferential focus on childhood obesity -- by researchers, and by my journal, is, of course, well justified.

As I have noted on many prior occasions, obesity and overweight are rampant among our children. The official definition of obesity in children (age -- and sex -- adjusted BMI at or above the 95th percentile) is willfully exclusive, rather than inclusive, to help minimize the numbers of children subject to the stigma of the term. By any reasonable definition, I think half of the children nationwide are already affected.

What was, a generation ago, "adult onset" diabetes, now routinely affects children under age 10 -- and is euphemistically called "type 2" diabetes, rather than what it really is: adult onset diabetes occurring in childhood due to massive societal irresponsibility and failure.

Studies show ever more cardiac risk factors in ever younger people. And, at the American Stroke Association's International Stroke Conference held in Los Angeles this year, a marked increase in the rate of stroke in children ages 5 to 14 was reported, and attributed to the obesity epidemic. QED!

My own work with school-based programming
has revealed to me that kids may be motivated to eat better, then go home to parents worried about -- and opposed to -- the potentially higher cost of more nutritious food (this worry is somewhat unfounded, by the way!). My work counseling adults has revealed again and again how challenging it can be to be active and eat well when the children at home vote no.

So there is danger in failing to see through the people to the family, as there is with the forest and its trees. What of opportunity?

Well, here is one simple idea that occurred to me. Among the many challenges schools face in efforts to adopt well-considered health promotion programming is lack of money. Worksites tend to have money, and more and more of them are spending some of it on wellness and weight control because they really have no choice. Just as companies adopt stretches of highway to demonstrate good corporate citizenship, and get some good PR by posting a sign that links a lack of litter to their company name -- they could adopt schools.

In fact, chambers of commerce could systematically help link companies to schools so that the schools get sponsored wellness programming; the companies get the recognition they deserve; and the health promotion efforts for kids in any given school or district are complementary to the programming for the parents of those very kids at a given worksite. And reciprocally, school-based health programming can be designed so it reaches into the home, and empowers parents too.

We all know what Lincoln told us about a house divided. I think it pertains no less well to the pursuit of health. Parents motivated to pursue the happiness of better health may be fully sabotaged by disengaged kids who don't want to play. Kids, inspired and enlightened by a school wellness program may hit an unyielding wall in parents opposed to change.

The basic functional unit of our society is ... the family. The household. No one of us is an island; and in our unity, there is potential strength.

Ideally, a whole array of programs and policies will help bridge the divide between kids and adults currently seeking health and weight control along separate tracks. But while waiting for this to be the norm rather than the exception, we can take matters into our own hands. We can make health a family value under our own roof -- and unite the house.

saw an intersting news story last night where this man hypnotises people to believe they have had gastric band surgery
there was an image of the participants throwing 50kg of fat onto a pile, it was enormous, it made me think that these women were carrying a lot of weight that somehow didn't seem as big as the stuff they were throwing on the pile.
the hypnotist worked with a nurse who had been involved in thousands of lapband surgeries. she helped create the monologue of the hypnotist.
this man has been working for some time doing this and they interviewed some of his success stories

In the counseling profession, "systems theory" views a child's disorder as really a disorder of the system: the family (and school, community, etc.). Parents often want a child "fixed", but the family is the one that is disordered, and when the family is treated, the child gets better. If the family is not treated, attempts to "fix" the child without change in the system are futile. This is the basis of family therapy. In the same vein, seeing childhood obesity as a problem apart from our family and cultural obesity disorder is flawed. So, you are absolutely right, and clinicians trying to treat childhood obesity need to be led in the direction you are going. Good work!

Because "The Anderson Method" has been so successful in helping adults to acheive lasting success in weight loss, I am frequently asked if it is appropriate for children, with the childhood obesity epidemic receiving the attention it has. My answer is always the same, in line with systems theory: children are a part of a system, and for the part to get better, the system must be treated. You have taken on quite a mission. My answer is always the same, in line with systems theory: children are a part of a system, and for the part to get better, the system must be treated. I struggle to find a pithy and actionable response to this post that would arm every parent to do what's best for his/her child. But this issue of childhood obesity is complicated and related to more than just food, such as a generational sessileness ("being directly or permanently attached" to the couch, to the online game, etc.).

Moreover, the 66% of Americans who are overwieght don't clearly know how to slim down, let alone how to get their kids to do so.

This issue is going to bankrupt our health care system and ruin the quality of many, many lives!

When I was a kid, obese kids, unlike today, were rare. We had the benefit of running free in the neighborhood and playing/exercising outdoors from daylight till dark. Many kids miss that freedom today. So the role of parents in their children's diet is even more important and it's got to start early -- before kids can pick up bad eating habits:

Eat less and exercise. Have your children eat less and exercise. Weight is entirely about the number of calories consumed minus calories burned. Take control of your life and encourage others to do the same.

Thing is, eating less is not even an issue if these kids are eating healthy, natural foods. They will be so full of energy, that they would naturally burn off any excess. It's tough to go McDonalds and load up on junk then ask your body to go burn it off, when all it wants to do is rest from all the excess work the kidney, liver, pancreas, digestive system etc. has just been asked to do. I swear, when I was in 6th grade, I only remember one maybe two fat kids in the whole school!

...weight loss. Here's a study that suggests so: women who exercised 45mins 3x a week compared to women who didn't exercise. After one year, those who exercised only weight 4Lbs less. Not much of a difference.

If it were that easy, people would be partially starving themselves (aka restricting calories) and losing weight no problem, right? The cals in/cals out model has repeatedly been shown not to work for most people long term, because they tend to just eat less of the crappy foods that got them overweight.

The types of foods we eat, and more importantly don't eat, will manage weight far better than telling people to eat smaller portions of ice cream and cake.

Also, exercise isn't key to weight

Thanks for the insight! Everything is connected - mind/body, kids habits/parents habits. Make getting healthy a family affair. Government guidelines for kids say to aim for 60 minutes of activity every day. A game outside, a bike ride, hike, or some yard work could get an hour to fly by. If there is time for TV, the internet or video games, there is time for exercise. ---Oh, you should get about an hour of movement a day too. Lets face it. It requires constant vigilance to avoid becoming fat.

Our society has become obesitogenic.

When I walk into my store, I have to hunt to find the REAL foods hidden beneath mounds of prepackaged junk.
Our communities aren't walkable. There is no shopping to walk to.
Our kids don't play outside anymore out of fear.
Our foods are filled with CORN SYRUP, in fact the bulk of our diet is corn.
We eat grains that have been denuded of their proteins.
Most of us do sedentary work.
Even jobs in construction have become less physically demanding.

This is a complex issue. That will require action on many fronts. It demands wise public policy and active implementation of remedies.

This is SOOOOOOOOOOOOOOOOO true....we have to really hunt to find the food and snacks with the good ingredients.............. It is so frustrating... I mean I can go into a health food store and know that all the items there are made without the bad sugars and oils, as well as having the healthy ingredients as well... problem is I cannot afford to shop there...so I go to grocery chains and get the bad stuff... then we feed it to our kids, we know it is bad for them, but what can we do? I mean when will these big food companies ever GET IT?? That kids need healthy food? Healthy ingredients (and no my kids will not eat fruit all the time, they do need to eat snacks..) We really need to completely redo how are culture PREPARES food and what they offer.... We need to start stealing ideas from the other cultures which DO eat really healthy, all the time.... i.e. Italy, Greece, India....

We don't need public policy, just heightened awareness. Food is a huge industry so don't expect too much from politicians or public administrations. Those who know need to spread the word as much as possible.

There are new ideas that suggest a pattern of both food in and waste out is set up in childhood. This is set up during the breast feeding and then weaning periods of infant growth. Too much food in and too little weaning will not only lead to obesity,but also a lack of independence (from the weaning part).
There is a new hypothesis that suggests that each infant sets up a 3 part digestion system that may play a key part in every aspect of his life (including both overweight and underweight problems).*
He develops, over the first years of his life a pattern of a) breaking down food into usable nutrients - from mouth to stomach, b) absorbing those nutrients - in small intestine, and c) excreting out waste - from large intestine.
Therefore each child sets a 3 part pattern in infancy of how he will digest food. This seems to subconsciously program almost all behavior from then on.
This pattern is in the ENS, or Enteric Nervous System or digestion brain. It is unconscious motivation, and it is very difficult to change.
These three patterns are set up through breast feeding and weaning.
Generally problems with breast feeding will lead to overweight problems, and problems with weaning will lead to underweight problems.
Resetting this 3 part infant digestion pattern in the ENS, solves a vast layer of human physical and psychological problems that seems to be quite extensive. *This is an untested hypothesis at this point.

i have three kids and the slimest was actually a heavy weight baby that has eaten whatever he want. never forced to eat more than he wants and always given food when he wanted it
at age about 4 i noticed that when he starts to eat a meal he will need to eliminate from his bowels. then after he eliminates he goes back to eat but then of course he already has some food in his stomach and the time spent on the toilet means his brain is now telling him that he has had enough food and he rarely finishes the dinner.

*applauds* As a parent, I whole heartedly agree with the sentiment in this article. Unfortunately, we have an entire generation of parents who don't know how to cook, claim they don't have time, and don't take the effort to read ingredients on their food. And the food corporations are entirely complicit in this - I can't count how many times I've seen something marketed as "healthy" and "real fruit juice", only to note that it has almost no nutritional value whatsoever.

My family had a birthday party a few weeks ago. Some of the kids had dietary restrictions, so I baked a separate cake for them to eat. Most of the parents chose to give their children the Costco cake, but several of them tried the home-made cake and commented that it tasted so much better than the commercial mix cake from Costco. The PARENTS seemed surprised by this. When adults are shocked and surprised that something made with comprehensible ingredients, real sugar, real eggs, real milk...tastes BETTER than something made with reprocessed milk powder and HFC...It's a very telling symptom of what's really wrong with the food culture.

We have been indoctrinated to believe that unhealthy, cheaply made, chemical-filled food is better. That convenient, fast and non-perishable is more important. It will take a complete redesign of our nutritional education programs to fix this. Can that be pushed through voter lethargy AND corporate objections?

Have any of these parents tried using the word "no?" And if no doesn't work, well these parents probably lost most of the control they should have over their childrens' behavior a long time ago.

Or get creative. Buy the box with Sponge Bob on it and swap out the contents inside for whatever you actually want in there and just keep reusing that box. Either get tough, get creative, both, or get lost.

Alternatively, don't let your kids watch Sponge Bob in the first place because it's a ridiculously asinine immature show fit only for rotting children's brains. Just saying. :)

"No" is a very good word. I like it a lot. It has wonderful effects on small children. I can't understand why more parents won't use it when their kids are making absurd demands.

You said exactly what I was thinking. Kids may whine and beg for the bad stuff but you are the parent and the word 'no' shouldn't be outside your vocabulary. Or go ahead and give in once in awhile, after all, there is nothing wrong with the kids having a treat here and there so long as it doesn't make up the bulk of their diet. They'll even enjoy it more when it is the exception rather than the rule.

I see this problem within my classroom. Very sad indeed and yes, it usually goes back to the family. I have yet to see a morbidly obese child with thin parents.
