Stopping the AIDS Epidemic in Its Tracks

World AIDS Day is a time for us to consider the state of the epidemic and the challenges we must overcome to achieve a world without AIDS. It's a time to reflect on the fact that we ALL have a role to play in ending this disease. And one of the most important ways we can stop AIDS in its tracks is simply by fighting stigma and homophobia.

This World AIDS Day nearly coincides with the 20th anniversary of the death of my dear friend, Freddie Mercury. If Freddie were alive today he would feel very much as I do. He'd be astonished by how far we've come in treating and preventing HIV/AIDS since the frightening and tragic early days of the epidemic. But he'd also be saddened and dismayed to see that rampant stigma and homophobia continue to drive this disease.

The devastating impact of discrimination against gay people and people living with HIV are clearly reflected in the alarming incidence of HIV/AIDS in the gay community. In American cities, as many as one out of every five gay and bisexual men is HIV-positive, and half of those infected are unaware they have the disease. Indeed, HIV prevalence in our community is on par with some of the hardest hit regions of the developing world. Also, new HIV infections are actually on the rise among gay and bisexual men -- the only risk group in America for which this is the case.

Clearly, we must do more, MUCH more, to reduce the incidence of HIV among gay and bisexual men, and that work has to begin within our community. In the early days of the AIDS epidemic, we rose up with our straight allies, claimed for ourselves equal rights and equal value as human beings, and demanded solutions for a health crisis that affected not just the gay community but every population across the globe. And it WORKED! HIV rates among gay men declined dramatically by the late 1980s, and we can be justifiably proud of our efforts then.

But today's statistics show that we have stalled in our drive against this disease. We've dropped our guard and become complacent, and in that void, AIDS is thriving in our community once again.

Today, on World AIDS Day 2011, I'm ringing the alarm bell. We must WAKE UP! There are three immediate challenges before us, and we have to address them NOW!

First, we must help our young people to combat the many negative messages our society still flings at gay people. We have to teach gay men to love and accept themselves, to value and protect their health and the health of others, and to join the campaign for our equal rights as human beings. We cannot be silent or invisible. The old slogan "Silence = Death" is every bit as relevant today as it was in the 1980s. Homophobia can be neutralized by familiarity and experience and compassion. Stigma can be eradicated by courage and pride and unity. We can begin to end AIDS when we empower ourselves.

Second, we must take responsibility for our own health and well-being. We must get tested and retested. Too many of us do not know our HIV status, and that MUST change.

Third, we must not let our federal and state governments balance their budgets by cutting crucial funding for HIV prevention, treatment, and research. Reducing or eliminating HIV programming today will cost us much MORE money down the road. That's because these investments pay for themselves in terms of infections prevented, health preserved, and lives saved. Earlier this year, the National Institutes of Health released a groundbreaking study demonstrating conclusively that people living with HIV who receive effective antiretroviral treatments are 96 percent LESS likely to pass the disease to their sexual partners. In other words, HIV treatment IS prevention. Therefore, we should be INCREASING funding for HIV treatment programs, not implementing cuts, as many states are doing today.

We have all of the tools we need to stop this epidemic in its tracks. Working together, I believe my little son Zachary and his generation can live to see a future without AIDS. But to get there, we have serious work to do. We must fight stigma, homophobia, and apathy. We must learn to love and value our lives and our health. We must be honest in our own relationships. We must get serious about the risky behaviors that have become commonplace once again in our community, and the negative messages that encourage this behavior. We must acknowledge the dangerous substances that are known drivers of infection. We must demand health funding.

But more than anything, we must educate and mobilize young people to join the fight not only AGAINST the AIDS epidemic, but also FOR health and acceptance and love. On this World AIDS Day, let us spread messages of tolerance and compassion that are so critical to ending AIDS.

No, fighting homophobia will do nothing to combat AIDS........ some of this article was good, other parts were complete BS. The rates are higher amongst gay men because anal sex has a higher transmission rate than other forms of intercorse -- NOT because of "stimga". (and yes, I am a health care professional)

There is only one thing that will stop AIDS, the same thing that did polio and small pox. That is what is needed, not compassion and understanding. Sure, they are great things to have, but they will not same lives!

We see lots of adverts that smoking kills and how it affects your health but we do not see this sort of advert for AIDS ,AND YET IT MORE AND MORE ARE BECOMING AFFECTED BY IT ,so lets see goverment adverts on this ,like ,how you can catch it ,how you can protect yourself against it ,and how there is no cure for it,and if safe sex is being tought at schools this all should be explained as well

AIDS could have been stopped in the first year or so, with commonsense screening and quarantine. But gay liberationists intimidated the SF health department into letting them continue to romp in the bathhouses, with deadly consequences.

In 1981 there was no test for HIV. A test did not come along until 1985. By the time people were dying of this disease there were 10's of thousands already infected probably dating back to the early to mid 1970's since this disease has about a 5 to 10 year period until symptoms show up. The federal government did not want to deal with this epidemic and as a matter of fact the only thing the federal government wanted to do in the 80's was build more nuclear weapons for the cold war and cut public health spending to the bone. Nothing was going to stop this disease unless you stopped people from having sex and that was not going to happen. This is not a gay disease just ask all the straight black women that have been infected. The gays were just the unlucky group to get infected first.

It was not that MSM were unlucky. Far from it. And you can take this from a Public Health professional who was working inside the baths and other venues at that time.

It was because many MSM were having a very very large number of sex partners. Numbers that straights will have a hard time understanding.

And they were doing certain acts that made the transmission of HIV a lot easier. Acts that straights as a group do not do.

Unfortunately you cannot name those acts here and get your post to show up.

In the late 70's and early 80's it was set up as a Prefect Storm as far as the MSM community and the transmission of HIV.

And people did not need to stop having sex. Just as they do not need to stop today.

They needed to cover up.

Even the most passionate supporters of the bible seem to forget "judge not, lest ye be judged." It's not realistic to expect all humans to prescribe to one viewpoint.

What is possible, with more awareness and, most importantly, responsible, safe sex, is the gradual decline of HIV amongst the population. Americans need to focus on sex education and also need to hammer the point home to citizens of all sexual orientations that unsafe sex is inherently risky. Gay sex does not have to lead to disease of any kind. There are plenty of healthy, elderly gay people who've had active sex lives to testify to that.

More important than god, more important than the government, more important than Elton John, is the will of individuals. This crisis will stay until people start treating sex responsibly and as long as rapes are allowed to occur at the rates they do in countries like South Africa.

Good article! Thank you Sir Elton John for supporting a good cause. It's time for the insurance companies to allow HIV positive patients to become vaccinated against this horrific virus. A lot of times the meds are available, but insurance will not cover the cost. Then what? Some people cannot afford to pay for their meds. So they have to suffer. Not fair at all!!! And BIG PHARMA needs to release these preventative meds and make them available. I don't mean to get off the subject here, but whatever happened to our antibiotics??? I had to beg my doctor to give me an antibiotic for my bronchitis. The good old fashion penicillin shots and pills. They worked! Is BIG PHARMA holding out on us in case of some kind of chemical warfare? And the antibiotics that really work will only be available to the ones who can afford to pay for them? Makes you wonder?????? Anyways, thanks again for this article--awareness is a key in getting the help these people need.

Antibiotics and anti-virals have been over-used and have actually become detrimental to our immunological health. It is good for us to have the common cold. It's just a week of dealing with crud. It is good to play in the dirt; especially as a kid. The alternative is to live in bubbles.

Simply wising it away won't do JS as far as preventing the spread of AIDS. Stigmas have absolutely nothing to do with this. It's a disease, not a thought. Ironically, it's those big businesses that the youth of today are so against existing, that will cure the disease, not simply wishing it away or changing it's name. It's the same large pharmaceutical companies that have come up with ways to keep those infected alive longer and the only thing that folks like the OWS'ers can do is protest against them. Funny, isn't it?

I assume you are neither a public health worker nor an historian.

I can't begin to count the number of truly deadly diseases that are now prevented because Big Pharma and Big Government worked together and put those diseases down for the count.

The very thing you claim is not being done.

Small pox for one. Polio for another. We could be hear all day making a list but you get the point I assume?

Seen any cases of small pox recently? I thought not.

I also encourage educators to teach sex protection and not just abstinence- we can not continue to miseducate our youth by endangering them and not giving them the proper knowledge and heath protection.
